ACCESSCareLink

ACCESSCareLink is an online portal that extends ACCESS' Epic electronic health record to affiliate community partners to enable easy access to shared patient information. This enhances communication between ACCESS and our partners while allowing for easy follow-up and continuation of care.

Access to this platform must be granted directly by an ACCESS team member.

If you and your organization are interested in enrolling in ACCESSCareLink, please email accesscarelink@achn.net with the following information: 

• Name of organization
• Point of contact information
• How you heard about ACCESSCareLink
• If you are an Epic user or customer
• Reason for request
