Good Faith Estimates

At ACCESS, we believe in cost transparency and your right to shop for high-quality, cost-effective health care services. If you do not have health insurance or a plan to self-pay for services received at an ACCESS location, we will be happy to provide you with what is called a Good Faith Estimate of our cost(s) at least 3 days after your appointment is scheduled.

  • Under federal law, health care providers must provide patients who do not have insurance or are not using insurance, an estimate cost for medical items and services. This is called a Good Faith Estimate
  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests and health center fees.
  • ACCESS will provide a Good Faith Estimate in writing before any scheduled medical service. For certain types of services, a clinical evaluation may be required to provide you with an accurate Good Faith Estimate.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate or have additional questions, you can contact the ACCESS Customer Service at 312.526.2424 or customerservice@achn.net.
  • For more information about Good Faith Estimates or if you want to file a formal dispute, please call 800.985.3059 or click here.
