Who can participate in Advanced Primary Care Management?
Advanced Primary Care Management is available to eligible Medicare Part B and Medicare Advantage patients who receive primary care at ACCESS.
Eligibility and program requirements may vary. Your ACCESS care team can help determine whether you qualify and explain what participation in the program means for you.
What happens after I enroll?
After you consent to participate, you will be connected with an ACCESS Care Coordinator. Your Care Coordinator will work with you, your PCP and other members of your care team to understand your health needs and goals.
Your Care Coordinator may contact you between appointments to:
- Check in on your health and care needs.
- Help coordinate appointments and services.
- Support medication management.
- Follow up after a hospital or emergency department visit.
- Connect you with health education and community resources.
- Help you stay connected to your ACCESS care team.
The goal is to make your care easier to understand, easier to manage and centered on what matters most to you.
How can Advanced Primary Care Management help me?
When you consent and enroll in the program, you will be assigned to a Care Coordinator who works with you, your primary care provider (PCP) and your care team. Your Care Coordinator can help you understand your care, stay on track with your health goals and make it easier to navigate the health care system.
Your Care Coordinator can provide personalized support based on your health needs and goals, including:
- Coordinate your care: Help schedule medical appointments, arrange transportation and connect you with your care team.
- Manage your medications: Help you understand your medications and communicate medication-related questions or concerns to your care team.
- Stay on track with your health goals: Help you understand your care plan, manage your health needs and follow up on recommended care.
- Follow up after a hospital or emergency department visit: Help you understand next steps and coordinate follow-up care after you leave the hospital or emergency department.
- Connect you with health education and resources: Help you find information and community resources that support your health and well-being.
- Stay connected with your care team: Connect with your providers and care team through myACCESSHEALTH, ACCESS' secure online patient portal.

How much is it to enroll in Advanced Primary Care Management at ACCESS?
APCM provides additional care coordination and support between visits. Depending on your insurance benefits, a copay may apply. Our team can answer questions about the program and connect you with a Patient Benefits Specialist to review your coverage and discuss programs that may be available to help with costs.
