Keston John, M.D.

Keston John earned his medical degree at St George’s University School of Medicine in Grenada and completed his residency at Mt. Sinai Hospital in Chicago. He is a licensed physician in both Illinois and Iowa and is Da Vinci Robotic Surgery Certified. Dr. John is a member of the American Congress of Obstetrics and Gynecology, the American Association of Gynecologic Laparoscopists, and the American Medical Association. He strongly believes in equitable access to health care and works to build lasting relationships with his patients. Dr. John's philosophy of care is that a physician serves first and teacher, second. He is committed to serving his patients and educating them on how to live a healthy lifestyle. He continues to give back to the community by presenting at health fairs for underserved communities in his home country of Trinidad and Tobago. Outside of work, Dr. John enjoys Formula 1 racing, scuba diving, traveling, and continuing his education.  

Linda Guzman-Kurani, N.P.

Linda Guzman-Kurani received her Master of Science in Nursing from the University of Illinois at Chicago. She is licensed by the Pediatric Nursing Certification Board and is a member of the National Association of Pediatric Nurse Practitioners. In her spare time, Linda enjoys traveling, dancing, swimming and testing her taste buds with new foods. Linda grew up receiving her primary care through community health, so understands first-hand the impact it can have on a patient. She is dedicated to listening to her patients, advocating for them and providing the best possible patient care. Linda is fluent in Spanish.  

Luke Agase, P.A.-C.

Luke Agase completed his Masters of Physician Assistant Studies at Lipscomb University in Nashville, Tennessee. He is credentialed through the National Commission on Certification of Physican Assistants and is a member of the American Association of Physician Assistants and the Illinois Academy of Physician Associates. In his practice, he seeks to serve people of all backgrounds with compassion and kindness. He is committed to ACCESS’ mission of providing high quality, patient centered care to all of his patients. Outside of work, Luke enjoys building computers, spending time in record and coffee shops, and spending time with family and friends.  

KaSandria Clark, M.D.

KaSandria (Kassy) Clark completed her medical degree at the University of Illinois College of Medicine and completed her residency at Advocate Children’s Hospital in Park Ridge, IL. She is a Board-certified physician and is also a Certified Lactation Counselor through the Academy of Lactation Policy and Practice and a member of the American Academy of Pediatrics. Dr. Clark is passionate about serving her community and knew she wanted to dedicate her career to improving their access to care. She treats every family and child as if they were her own and promises to help educate families on newborn care and breastfeeding. She is dedicated to helping families navigate school accommodations and therapies for their children so that they can succeed academically. Outside of work, Dr. Clark enjoys playing tennis, hot yoga, reading fantasy novels, and live music.  

Muhammad Shahzeb Najam, M.D.

Shahzeb Najam graduated from Ziauddin University in Pakistan and completed his internal medicine residency at Weiss Memorial Hospital in Chicago. Dr. Najam is board-certified by the American Board of Internal Medicine, the National Board of Physicians and Surgeons, and is a member of the American College of Physicians. He is an evidence-based clinician who continually educates himself on best practices and new technology within the medical field. Dr. Najam treats his patients like family and makes sure that they feel heard and that all of their needs are met. He works with patients to create individualized plans that meet their health goals. Outside of work, Dr. Najam is an avid reader and guitar player. He is fluent in Urdu. 

Marie E. Schlentz, D.O.

Dr. Marie Schlentz received her medical degree from Chicago College of Osteopathic Medicine and completed her residency at Rush-Presbyterian St. Luke Medical Center, Chicago. Dr. Schlentz is a member of the American College of Physicians and is licensed by the Illinois chapter of the American Board of Internal Medicine. She is dedicated to a practice foundation of strong patient-provider relationships and preventative medicine. Dr. Schlentz encourages patient understanding, lifestyle changes, and compliance with the proper and least amount of medication for treatment. ​She is an avid reader and in her free time enjoys scuba diving, aerobic exercise with weight lifting, travel, and an overall healthy lifestyle. 

Nasreen Shah, M.D.

Dr. Shah was born and educated in Pakistan and has since become a US citizen. Her degree is from Islamia University in Pakistan and she also completed two internships there - one in the Surgical Department of Baqai Hospital and one in the Department of Medicine of Civil Hospital. From 1992-1994, she served as a resident in internal medicine at Ravenswood Hospital and Medical Center in Chicago. Dr. Shah is a member of the Pakistan Medical Council.

Kinga Radomski, F.N.P.

Kinga Radomski earned her Master of Science in Nursing–Family Nurse Practitioner from Lewis University and is certified through the American Nurses Credentialing Center (ANCC) and is a member of the American Nurses Association and the American Association of Nurse Practitioners. Kinga is committed to providing accessible, high-quality, patient-centered care to individuals and families from all backgrounds. She believes everyone deserves compassionate care, regardless of their circumstances, and is dedicated to supporting the health and well-being of her community. Her philosophy of care emphasizes listening closely to patients, ensuring they feel informed and involved, and working collaboratively to support long-term health. She values patient education and partnership as essential components of effective care. Outside of work, Kinga enjoys cooking, taking on home improvement projects, gardening, walking her dogs, and spending time with family and friends. 

Grace Olatunji, W.H.N.P.

Grace Olatunji graduated from the University of Illinois at Chicago with a Doctor of Nursing Practice (DNP) degree and is a credentialed Nurse Practitioner through the National Certification Corporation. Grace was drawn to ACCESS because she wanted to work at a health center that provided quality care to women in medically underserved communities. As a Women’s Health Nurse Practitioner, Grace’s goal is to provide culturally competent and accessible care to women, with an emphasis on their bodily autonomy and informed decision-making. Outside of work, she enjoys cooking, watching timepiece shows and movies, and hosting game nights with friends.

Bushra Ali, M.D.

Dr. Ali earned her medical degree at the University of Jordan School of Medicine and completed her residency at the University of Chicago. She is a Board-certified pediatrician by the American Board of Pediatrics and is a member of the American Academy of Pediatrics. Dr. Ali believes that as a health care provider, it is her mission to provide fair, equitable, and high-quality care to people of all backgrounds. She understands that no single approach fits all patients and takes the time to understand each patient’s concerns from their perspective. Outside of work, Dr. Ali enjoys reading, journaling, cooking, and gardening. She is fluent in Arabic.

Chelsea Schroeder-Lozada, D.N.P.

Chelsea Schroeder-Lozada earned her Doctor of Nursing Practice degree from the University of Michigan. She is credentialed through the American Nurses Credentialing Center and is a member of the American Nurses Association, Michigan Council of Nurse Practice, and the Illinois Society of Advanced Practice Nursing. Chelsea is passionate about partnering with communities to improve health outcomes and advance health equity. She is dedicated to providing compassionate, patient-centered care that empowers her patients to take an active role in their health. Additionally, she is deeply interested in global health and completed her doctoral project within HIV positive communities in the Dominican Republic. When not at work, Chelsea enjoys spending time with friends and family, reading, and traveling.  

Mario Ortiz, P.A.

Mario Oritz completed his Master of Science in Physician Assistant Practice at Rosalind Franklin University and is credentialed through the National Commission on the Certification of Physician Assistants. He chose to work in a community health center because he believes health care should be accessible to everyone, regardless of patients’ insurance status or socioeconomic background. He treats his patients the way he would want his family to be treated. As a husband and father of three, he understands how important it is to build strong, quality relationships with his patients. He treats each patient with kindness, respect, and dedication so that they are empowered to make the best decisions for their health and well-being. Outside of work, Mario enjoys spending time with his family, staying active outdoors, and playing board games. He is fluent in Spanish.  

Lauren Mann, P.A.

Lauren Mann earned her Masters of Physician Assistant Studies from the University of Nebraska Medical Center. She is credentialed through the National Commission on Certification of Physician Assistants and is licensed through the state of Illinois. Lauren chose to work in a community health center to help improve health equity in Chicago and give back to the community. She takes a patient-centered approach to care and makes sure her patients feel comfortable and their families are confident in their care plans. Lauren was inspired to work in pediatrics by her mother, who is a pediatric home health nurse. Outside of work, Lauren enjoys jogging, reading, and embroidery.  

Kyunghoon Rhee, M.D.

Kyunghoon (Daniel) Rhee, MD earned his medical degree from Chungnam National University in Daejeon, South Korea and earned a Master's of Science in Health Systems Management at Rush University in Chicago. He completed his internal medicine residency at the Greater Baltimore Medical Center in Baltimore, Maryland. Dr. Rhee is Board certified through the American Board of Internal Medicine and is a member of the American College of Physicians and the American Board of Internal Medicine. Dr. Rhee is committed to advancing health equity through quality, data-informed care. As a provider, he works to build long term, trusting relationships with his patients and deliver evidence based, holistic care. Outside of work, Dr. Rhee can be found going to the gym, spending time with family and friends, or watching a soccer or basketball game. He is fluent in Korean.  

Zehra Kanji-Ghaziani, D.N.P., F.N.P.-B.C.

Zehra Kanji-Ghaziani received her Doctorate in Nursing Practice in Family Medicine at DePaul University and also holds a Masters of Science in Nursing. She is credentialed through the American Nurses Credentialing Center. Prior to receiving her doctorate, Zehra was a labor and delivery nurse. She is committed to providing patient-centered, holistic, and equitable care to all her patients. Zehra is drawn to community health care because she wants to ensure that all members of her community have access to care. When not at work, Zehra enjoys spending time with her husband, Esmael, and their dog, Railey. She is fluent in English, Hindi, and Urdu.
