Bridget Adams, PA-C, received her Master’s of Science in Physician Assistant studies at New York Institute of Technology and completed her residency in New York. Bridget is credentialed by the National Commission on Certification of Physician Assistants (NCCPA) and is a licensed physician assistant in Illinois. She is also a member of the American Academy of Physician Assistants. Bridget believes in providing exceptional, individualized care to her patients and chose community health care to positively impact the lives of patients and communities. Bridget played basketball through college and enjoys spending time with family and exercising.

Angeli Patel, N.P.

Angeli Patel received her advanced practice registered nursing degree and completed her residency at Loyola University Chicago. Angeli is credentialed by the American Association of Nurse Practitioners. Angeli is drawn to community health because she believes in helping people by promoting health and wellness to prevent disease. In her free time, Angeli enjoys working out and spending time with family and she is fluent with Gujarati.

Chirag Patel, N.P.

Chirag Patel received his Master of Science in Nursing from DePaul University in Chicago where he also earned a post-master's certificate in advanced practice nursing. He is licensed by the American Nurses Credentialing Center and a member of the Illinois Society for Advanced Practice Nursing, the Illinois Nurses Association, and the American Nurses Association. He is dedicated to serving the needs of the community and providing high-quality care in order to improve individual’s wellbeing. As a provider, it’s important to Chirag that he develops a respectful and trusting partnership with patients in order to help them make informed decisions toward healthier lifestyles. Chirag is fluent in both Gujurati and Spanish.  

Briana Trainor, M.D.

Dr. Trainor received her medical degree from Southern Illinois University School of Medicine and completed a residency at Rush University Medical Center. She is a member of the American Academy of Pediatrics. In her spare time, Dr. Trainor enjoys travel, reading, listening to music, and spending time with family and friends. Dr. Trainor is fluent in Spanish and is passionate about working in a medically underserved community. She is committed to watching her young patients grow up to be happy healthy young adults.

Sarah Verbeek, P.A.-C.

Sarah Verbeek completed the Physician Assistant program at Rosalind Franklin University/Chicago Medical School. She is licensed by the National Commission on Certification of Physician Assistants and is a member of the American Academy of Physician Assistants and the Illinois Academy of Physician Assistants. Sarah enjoys working in a community setting because she can offer holistic as well as preventive care to her patients. In her spare time, Sarah enjoys photography, travel, and tennis. She is also fluent in Spanish.

Susana Alvarez, M.D.

Dr. Alvarez earned her medical degree from the Universidad Francisco Marroquin in Guatemala and completed her residency at Howard University Hospital in Washington D.C. She is a member of the American Academy of Pediatrics and is fluent in both Spanish and French. Dr. Alvarez focuses on community involvement in her practice and outside of work enjoys learning other languages and sports.

Pinaki Patel, D.O.

Dr. Patel received her medical degree from Kansas City University of Medicine and Biosciences and completed her residency at Golisano Children’s Hospital in Rochester, NY. Dr. Patel is a member of the American Academy of Pediatrics and the American Osteopathic Association. She believes that community-based health care provides a greater, much needed service of health education and preventive care. Dr. Patel focuses her time with patients on offering measures that they and their parents can take to prevent the onset of disease. As a pediatrician, she enjoys guiding children throughout their growth and development. Dr. Patel speaks fluent Gujarati and, when not working, she enjoys cooking and baking, spending time with family and friends, trying out new restaurants and attending sporting events. 

Henry Innocente, L.C.S.W.

Henry Innocente received his Master’s in Social Work at the Dominican University and completed his internship at Heartland Alliance. He is licensed through the Illinois Department of Financial and Professional Regulation. Throughout his career, Henry has dedicated himself to working at the community level to help people struggling with addiction and other mental illnesses. He believes in empowering his clients to develop healthy lifestyles that include highlighting their unique interests that positively impact their overall health. Henry enjoys reading, working out, and planning his next road trips.

Chelsea Schroeder-Lozada, D.N.P.

Chelsea Schroeder-Lozada earned her Doctor of Nursing Practice degree from the University of Michigan. She is credentialed through the American Nurses Credentialing Center and is a member of the American Nurses Association, Michigan Council of Nurse Practice, and the Illinois Society of Advanced Practice Nursing. Chelsea is passionate about partnering with communities to improve health outcomes and advance health equity. She is dedicated to providing compassionate, patient-centered care that empowers her patients to take an active role in their health. Additionally, she is deeply interested in global health and completed her doctoral project within HIV positive communities in the Dominican Republic. When not at work, Chelsea enjoys spending time with friends and family, reading, and traveling.  

Jovan Chandler, L.C.P.C.

Jovan Chandler received his master’s degree from Roosevelt College in Chicago. He is drawn to community health because he has a desire to give back and serve those around him. He feels that everyone has a voice and important perspective that deserves to be heard. Jovan’s approach to care is through consideration of the whole person and not just addressing the specific problem at hand. Outside of work, Jovan enjoys reading, traveling and sports.

Sonia Antony, M.D.

Sonia Antony received her medical degree from Government Medical College in Nagpur, India and completed her residency at Rush Children’s Hospital in Chicago. Dr. Antony is certified through the American Board of Pediatrics and is a member of the American Academy of Pediatrics. She believes in providing high quality, compassionate care to children while involving their families and guardians in their care. Dr. Antony chose to work at a community health center because she believes that all people, regardless of their background, deserve access to quality patient-centered care. Outside of work, Dr. Antony enjoys music, reading, and traveling. She is fluent in Spanish, Hindi, and Malayalam.

Jessica Munoz, M.S.W.

Jessica Munoz earned her Master of Social Work degree at the University of Illinois Chicago after completing her practicum at the Gads Hill Center and Community Counseling Centers of Chicago. She is a Licensed Clinical Social Worker through the Illinois Department of Financial and Professional Regulation. Jessica believes that every individual deserves access to mental health services as an essential part of one’s overall health care. In her practice, she provides a safe and supportive space for all individuals and empowers them to create a life they envision. When not at work, Jessica can be found trying new recipes, exploring new places, and spending time with her family. She is fluent in Spanish.
