Ryan P. Blake, N.P.
Ryan Blake received his Master of Science in Nursing from Resurrection University where he also completed his residency. He is a credentialed Family Nurse Practitioner by the American Nurses Credentialing Center and has dedicated his career to serving under-served communities. He strives to make patients comfortable by welcoming them with a smile and a positive attitude. Outside of work Ryan enjoys spending time with family, friends and his dog.
Ellen Seymour, N.P., M.S.N., M.P.H.
Ellen Seymour, N.P., M.S.N., M.P.H. received her Masters in Nursing from the Johns Hopkins University School of Nursing and Masters in Public Health from the Johns Hopkins University Bloomberg School of Public Health. She completed her clinical training and practicum at the Baltimore city Department of Public Health and the Johns Hopkins Hospital. Ellen is a credentialed Adult/Gerontology Primary Care provider through the American Association of Nurse Practitioners and is a certified HIV and Viral Hepatitis care provider. She is licensed by the state of Illinois and a member of the American Association of Nurse Practitioners. The American Academy of HIV Medicine and the Association of Nurses in AIDS Care. Ellen believes everyone should have access to quality health care that is individualized, non-judgmental and empathetic. Passionate about public health, Ellen is dedicated to the health and wellness of each individual so the community thrives. Outside of work, she enjoys traveling, reading and watching football.
Anelia Stamatova, P.M.H.N.P.-B.C.
Anelia Stamatova received her Master of Science in Nursing from University of Illinois at Chicago. She is a member of the American Association of Nurse Practitioners. Anelia strongly believes that everyone should have access to high-quality health care. As an ACCESS provider, her goal is to deliver high-quality care by collaborating with her patients, their families and caregivers, and focusing on preventative care. Anelia is fluent in Bulgarian. In her spare time, Anelia loves spending time with family and friends. She also enjoys reading, dining at new places, and traveling to a new places around the world.
Arlette Brown, M.D.
Dr. Brown received her medical degree from the University Of Illinois College Of Medicine at Rockford and completed her residency at Mt. Sinai Hospital. She is board-certified in Obstetrics and Gynecology. Dr. Brown is a member of the American College of OB/GYN, and the American College of Surgeons. She strives to provide excellent care and to help the community. When not working, Dr. Brown enjoys traveling, exercising and fashion design.
Tréondis Snodgrass, P.A.
Tréondis Snodgrass earned his Master of Science in Physician Assistant Practice from Rosalind Franklin University of Medicine and Science and is credentialed through the National Commission on Certification of Physician Assistants. He is a current member of the American Academy of Physician Assistants. During his training, Tréondis also received a certificate of Distinction for Excellence in Interprofessional Healthcare to deliver holistic care to his patients. Tréondis is a passionate advocate for his patients and takes the time to educate them on medical issues and create a care plan that works for them. He provides high-quality, patient-centered and culturally competent care to people of all backgrounds. Outside of work, he enjoys traveling, performing music, and exploring the arts scene in Chicago.

Ahmed Memon is a Psychiatric Mental Health Nurse Practitioner and is a member of the American Association of Nurse Practitioners. Ahmed is eager to provide behavioral health services to the Chicago community. He believes that all people deserve access to high-quality, patient-centered care no matter their background. Ahmed treats each patient with respect and works to empower them to make the best decisions for their mental health. Outside of work, Ahmed enjoys gardening, going to the gym, and traveling. He is fluent in Hindi, Urdu, Sindhi, and Balochi.

Amy Lilly earned her Doctor of Nursing at Southern Illinois University and completed her residency in St. Louis and Illinois. She is a certified Psychiatric Mental Health Nurse through the American Nursing Credentialing Center. Amy values the contributions of community health centers and the critical mental health services they provide to medically underserved communities. She believes that mental health is essential to overall health and takes the time with each of her patients to listen to their concerns and treat them with compassion and dignity. Outside of work, Amy enjoys travelling and spending time with family and friends.
Christina Govas, M.D.
Christina Govas earned her medical degree at the Saint James School of Medicine and completed her residency at Southern Illinois University. She is a Board-certified physician through the American Academy of Family Physicians and is a member of the Illinois Academy of Family Physicians. Dr Govas’ passion for patient advocacy goes far beyond the health center – she has partnered with community leaders to prevent the closure of hospitals in medically underserved neighborhoods. In her work, she uses a team-based and holistic approach to care that empowers patients to reach their goals. As a doctor, she leverages her knowledge and expertise to create a personalized health plan that matches a patient’s lifestyle and health goals. Outside of work, Dr. Govas enjoys travelling, cooking, and yoga. She is fluent in Greek.
Grace Olatunji, W.H.N.P.
Grace Olatunji graduated from the University of Illinois at Chicago with a Doctor of Nursing Practice (DNP) degree and is a credentialed Nurse Practitioner through the National Certification Corporation. Grace was drawn to ACCESS because she wanted to work at a health center that provided quality care to women in medically underserved communities. As a Women’s Health Nurse Practitioner, Grace’s goal is to provide culturally competent and accessible care to women, with an emphasis on their bodily autonomy and informed decision-making. Outside of work, she enjoys cooking, watching timepiece shows and movies, and hosting game nights with friends.
Kara Egan, P.A.
Kara Egan completed her Masters in Physician Assistant Studies at Dominican University in River Forest, Illinois and is credentialed through the National Commission on Certification of Physician Assistants. Kara believes in the importance of shared decision making and prioritizes education and health literacy with her patients. Her goal is to create strong relationships with her patients that establish trust and provide them the best quality care. Outside of work, Kara enjoys exploring nature, spending time with her cats, and playing chess.

JaMauri Warren completed his Masters in Physician Assistant Studies at Rosalind Franklin University of Medicine and Science. As a physician assistant, he is dedicated to providing high-quality, culturally competent care while empowering his patients to make the best decisions for themselves. Raynel is also a member of the Illinois Academy of Physicians Assistants. Community care is important to Raynel, and she has a passion for closing the care gaps in underserved communities. She strives to provide high quality, patient-centered care to all of her patients and works to maintain positive relationships that allow her patients’ voices to be heard. Outside of work, Raynel enjoys staying active through Pilates, traveling, and cooking.
Eleanor Teoh, D.O.
Eleanor Teoh earned her Doctor of Osteopathic Medicine medical degree at the University of New England College of Osteopathic Medicine and completed her residency at Loyola Medicine MacNeal Hospital. She is Board-certified through the American Board of Family Medicine and is a member of the American Academy of Family Physicians and the American College of Osteopathic Family Physicians. After working with patients who struggled to navigate the complicated health system, Dr. Teoh knew she wanted to work for a health network that provided resources and guidance to her patients. Dr. Teoh meets each patient with compassion and understanding and works to improve their overall health. She is particularly interested in women’s health, chronic health management, and performing procedures including Osteopathic Manipulative Treatment (OMT). Outside of work, Dr. Teoh enjoys cooking, dining out, hiking, and yoga.
