Amy Lilly, P.M.H.N.P.

Amy Lilly earned her Doctor of Nursing at Southern Illinois University and completed her residency in St. Louis and Illinois. She is a certified Psychiatric Mental Health Nurse through the American Nursing Credentialing Center. Amy values the contributions of community health centers and the critical mental health services they provide to medically underserved communities. She believes that mental health is essential to overall health and takes the time with each of her patients to listen to their concerns and treat them with compassion and dignity. Outside of work, Amy enjoys travelling and spending time with family and friends.

Ahmed Memon, P.M.H.N.P.

Ahmed Memon is a Psychiatric Mental Health Nurse Practitioner and is a member of the American Association of Nurse Practitioners. Ahmed is eager to provide behavioral health services to the Chicago community. He believes that all people deserve access to high-quality, patient-centered care no matter their background. Ahmed treats each patient with respect and works to empower them to make the best decisions for their mental health. Outside of work, Ahmed enjoys gardening, going to the gym, and traveling. He is fluent in Hindi, Urdu, Sindhi, and Balochi.

Tréondis Snodgrass, P.A.

Tréondis Snodgrass earned his Master of Science in Physician Assistant Practice from Rosalind Franklin University of Medicine and Science and is credentialed through the National Commission on Certification of Physician Assistants. He is a current member of the American Academy of Physician Assistants. During his training, Tréondis also received a certificate of Distinction for Excellence in Interprofessional Healthcare to deliver holistic care to his patients.  Tréondis is a passionate advocate for his patients and takes the time to educate them on medical issues and create a care plan that works for them. He provides high-quality, patient-centered and culturally competent care to people of all backgrounds. Outside of work, he enjoys traveling, performing music, and exploring the arts scene in Chicago.  

Lena Abu-Safieh, P.A.

Lena Abu-Safieh earned her Master's of Science in Physician Assistant Practice at Rosalind Franklin University and is certified through the National Commission on the Certification of Physician Assistants. Lena was born and raised on the South Side of Chicago and is excited to continue to serve her community through providing quality care and fostering community. It is her goal for her patients and their families to feel respected and empowered to collaborate on their treatment plan. She is fluent in Spanish and Arabic.

Mohammad Chaudhary, M.D.

Dr. Mohammad Chaudhary received his medical education in Pakistan and completed residencies both in Pakistan and at Cook County Hospital here in Chicago. Board-certified by the American Board of Pediatrics, Dr. Chaudhary is also a member of the American Academy of Pediatrics, the Illinois State Medical Society and the Chicago Medical Society. He serves as an attending physician at Rush Presbyterian St. Luke Medical Center Saint Anthony Hospital and Advocate Bethany Hospital and is an assistant professor of pediatrics at Rush University Medical Center. He is the recipient of several honors for his work with pediatric patients.

Annamma Mathew, M.D.

Dr. Mathew received her medical degree from the Medical College of Trivandrum, Karala, India and completed her residency in pediatrics at Christ Hospital Medical Center. A veteran pediatrician Dr. Mathew's philosophy of care is to provide comprehensive health care for children in need. Dr. Mathew is fluent in Malayalam and Hindi.

Bryan Mullaney, F.N.P.

Bryan Mullaney received his Doctor of Nursing Practice degree from DePaul University, where he received specialized training to become a Family Nurse Practitioner. He completed his clinical training at several ACCESS locations in Adult Medicine, Women’s Health, and Pediatric Medicine. Bryan is credentialed by the American Association of Nurse Practitioners, where he is also a member. Bryan believes the best health outcomes come out of relationships with patients built on trust, listening and understanding. Bryan enjoys spending time with family, biking, fishing and cheering on Chicago sports teams.  

Wesley Harmston, D.O.

Dr. Wesley Harmston received his medical degree from Midwestern University and completed his residency at Resurrection Medical Center. Dr. Harmston is board-certified through the American Osteopathic Board of Family Physicians. He is a member of the Illinois Osteopathic Association, the American Osteopathic Association and the American College of Osteopathic Family Physicians. Dr. Harmston believes in providing considerate care and thoughtful interactions with all his patients. His approach to care involves listening empathetically to all patients and taking their concerns and questions seriously. He is also great at thoroughly explaining medical issues related to Osteopathic and sports medicine to his patients. Dr. Harmston is drawn to community health because he values creating strong relationships with his patients in the community. Dr. Harmston is fluent in Italian. In his free time, he enjoys hiking and White Sox baseball.

Alberto Manalo, M.D.

Born in the Philippines Dr. Manalo earned his medical degree from the University of the East in Manila Philippines. He then completed his internal medicine residency at Michigan State University before returning to the Chicago area. He currently resides in Morton Grove and enjoys outdoor activities in his free time.

Sara Meyer, P.A.

Sara Meyer earned her master's degree in physician assistant studies from Southern Illinois University and completed her clinical training in Mattoon, Illinois at Sarah Bush Lincoln. She is licensed in the state of Illinois and is a member of the American Academy of Physician Associates. Sara provides high-quality, compassionate, and accessible care to her community. Outside of work, she enjoys reading, cooking, and art. 

Raynel Zaitona, P.A.

Raynel Zaitona earned her Master of Medical Science in Physician Assistant Studies at Midwestern University and is licensed through the National Commission on Certification of Physicians Assistants. Raynel is also a member of the Illinois Academy of Physicians Assistants. Community care is important to Raynel, and she has a passion for closing the care gaps in underserved communities. She strives to provide high quality, patient-centered care to all of her patients and works to maintain positive relationships that allow her patients’ voices to be heard. Outside of work, Raynel enjoys staying active through Pilates, traveling, and cooking.

Eleanor Teoh, D.O.

Eleanor Teoh earned her Doctor of Osteopathic Medicine medical degree at the University of New England College of Osteopathic Medicine and completed her residency at Loyola Medicine MacNeal Hospital. She is Board-certified through the American Board of Family Medicine and is a member of the American Academy of Family Physicians and the American College of Osteopathic Family Physicians. After working with patients who struggled to navigate the complicated health system, Dr. Teoh knew she wanted to work for a health network that provided resources and guidance to her patients. Dr. Teoh meets each patient with compassion and understanding and works to improve their overall health. She is particularly interested in women’s health, chronic health management, and performing procedures including Osteopathic Manipulative Treatment (OMT). Outside of work, Dr. Teoh enjoys cooking, dining out, hiking, and yoga.
