Brett Ballard, M.D.
Brett Ballard received his medical degree from the University of Cincinatti College of Medicine and completed his residency at Nationwide Children’s Hospital. He is Board-certified through the American Board of Pediatrics and is a member of the American Academy of Pediatrics. Dr. Ballard enjoys working with his team to provide quality care for children as they grow. He is dedicated to helping children live happy lives while developing healthy lifestyles that will follow them into adulthood. When not at work, Dr. Ballard can be found playing basketball, reading, and listening to music.
Bryan Mullaney, F.N.P.
Bryan Mullaney received his Doctor of Nursing Practice degree from DePaul University, where he received specialized training to become a Family Nurse Practitioner. He completed his clinical training at several ACCESS locations in Adult Medicine, Women’s Health, and Pediatric Medicine. Bryan is credentialed by the American Association of Nurse Practitioners, where he is also a member. Bryan believes the best health outcomes come out of relationships with patients built on trust, listening and understanding. Bryan enjoys spending time with family, biking, fishing and cheering on Chicago sports teams.
Claudelia Cano, F.N.P.
Claudelia Cano received her Master of Science in Nursing from Loyola University where she specialized in Family Nurse Practice. Claudelia is certified by the American Nurses Credentialing Center and has experience working in community health care settings at Alivio and Erie Family Health Center. She believes everyone deserves access to quality health care and should be treated with dignity and respect no matter their race, background or sexual orientation. She believes that as health care providers, it is our job to address and prevent health care disparities. Outside of work, Claudelia enjoys traveling, dancing, working out and playing with her dogs. She is fluent in Spanish.
Michelle Dormanen, C.N.M.
Michelle Dormanen received her Master’s Degree from University of Illinois at Chicago. She is certified by the American Midwifery Certification Board and is also a member of the American College of Nurse-Midwives. Michelle strives to provide quality, individualized health care to women of diverse backgrounds to meet their specific needs. As a midwife, Michelle values a woman’s right to make informed decisions.
In addition to caring for women throughout pregnancy and childbirth, she offers contraceptive, gynecologic and well-women screening services to women of all ages. Her goal is to support women and families in a way that fosters trust and confidence. Outside of work, Michelle enjoys listening to music, being active and spending time with her son.
Alok Rastogi, M.D.
Dr. Rastogi received his medical degree from Post Graduate Institute, Chandigarh, India. He completed a pediatric residency followed by a fellowship in Neonatal-Perinatal Medicine from Cook County Hospital. He is board-certified by the American Board of Pediatrics and its sub-board of Neonatal-Perinatal Medicine. He was the Chairman of the division of neonatology at Cook County’s Stroger Hospital before joining ACCESS. He has worked with the underserved population throughout his career. He is dedicated to improving the health of all children, and treating them with dignity and respect. Dr. Rastogi enjoys travelling, reading and learning new languages.
Linda Guzman-Kurani, N.P.
Linda Guzman-Kurani received her Master of Science in Nursing from the University of Illinois at Chicago. She is licensed by the Pediatric Nursing Certification Board and is a member of the National Association of Pediatric Nurse Practitioners. In her spare time, Linda enjoys traveling, dancing, swimming and testing her taste buds with new foods. Linda grew up receiving her primary care through community health, so understands first-hand the impact it can have on a patient. She is dedicated to listening to her patients, advocating for them and providing the best possible patient care. Linda is fluent in Spanish.
Annamma Mathew, M.D.
Dr. Mathew received her medical degree from the Medical College of Trivandrum, Karala, India and completed her residency in pediatrics at Christ Hospital Medical Center. A veteran pediatrician Dr. Mathew's philosophy of care is to provide comprehensive health care for children in need. Dr. Mathew is fluent in Malayalam and Hindi.
Mohammad Chaudhary, M.D.
Dr. Mohammad Chaudhary received his medical education in Pakistan and completed residencies both in Pakistan and at Cook County Hospital here in Chicago. Board-certified by the American Board of Pediatrics, Dr. Chaudhary is also a member of the American Academy of Pediatrics, the Illinois State Medical Society and the Chicago Medical Society. He serves as an attending physician at Rush Presbyterian St. Luke Medical Center Saint Anthony Hospital and Advocate Bethany Hospital and is an assistant professor of pediatrics at Rush University Medical Center. He is the recipient of several honors for his work with pediatric patients.
Alberto Manalo, M.D.
Born in the Philippines Dr. Manalo earned his medical degree from the University of the East in Manila Philippines. He then completed his internal medicine residency at Michigan State University before returning to the Chicago area. He currently resides in Morton Grove and enjoys outdoor activities in his free time.

Rabeeya Khalid, M.D.
Rabeeya Khalid earned her medical degree from the Dow University of Health Sciences. She completed her residency in Internal Medicine at the University of Chicago Northshore and her fellowship in Infectious Diseases at Loyola University Medical Center. She is a Board-certified physician through the American Board of Internal Medicine and is a member of the American College of Physicians and the Infectious Diseases Society of America. Dr. Khalid is passionate about serving the community and actively engaging with patients to make sure they receive the best care possible. Her philosophy of care is to provide holistic, patient-centered care through the practice of preventative and evidence-based medicine. When not at work, Dr. Khalid can be found swimming, traveling, and spending time with family and friends. Dr. Khalid is fluent in Urdu and Hindi.
Lisa Oldham, M.D.
Lisa Oldham earned her medical degree at Washington University, St. Louis and completed her residency at Rush University Medical Center. Dr. Oldham is Board-certified through the American Board of Obsterics and Gynecology and is a member of the American Medical Association, American College of Obsterics and Gynecology, and the North American Society of Pediatric and Adolescent Gynecology. Dr. Oldham was raised by civil rights activists and mentored by community physicians who taught her the importance of health equity and community involvement. She is committed to providing high quality, equitable care to all patients regardless of their background and looks forward to giving back to her community. Outside of work, Dr Oldam enjoys staying active, cooking, reading, and traveling.

Dr. Marie Schlentz received her medical degree from Chicago College of Osteopathic Medicine and completed her residency at Rush-Presbyterian St. Luke Medical Center, Chicago. Dr. Schlentz is a member of the American College of Physicians and is licensed by the Illinois chapter of the American Board of Internal Medicine. She is dedicated to a practice foundation of strong patient-provider relationships and preventative medicine. Dr. Schlentz encourages patient understanding, lifestyle changes, and compliance with the proper and least amount of medication for treatment. She is an avid reader and in her free time enjoys scuba diving, aerobic exercise with weight lifting, travel, and an overall healthy lifestyle.

Fran Locke Rosenberg is a graduate of Yale University and of the University of Illinois at Chicago and is a member of the American College of Nurse-Midwives. Originally from Troy, New York, Fran is married with four children and enjoys cooking hiking and spending time with her family.

Amy Lilly earned her Doctor of Nursing at Southern Illinois University and completed her residency in St. Louis and Illinois. She is a certified Psychiatric Mental Health Nurse through the American Nursing Credentialing Center. Amy values the contributions of community health centers and the critical mental health services they provide to medically underserved communities. She believes that mental health is essential to overall health and takes the time with each of her patients to listen to their concerns and treat them with compassion and dignity.
