Nadia Ilhardt, M.D.

Nadia Ilhardt earned her medical degree from Rosalind Franklin University of Medicine and Science and completed her residency at the University of Michigan. She is Board-Certified through the American Board of Family Medicine and is a member of the American Academy of Family Physicians. Dr. Ilhardt recognizes how confusing the American healthcare system can be and works with patients to navigate the system and provide high-quality care. She believes that everyone deserves patient-centered, holistic care in order to maximize their overall health. Outside of medicine, Dr. Ilhardt enjoys time with her family and spending as much time as possible outdoors.

Gabrielle Alessia, P.A.

Gabby Alessia earned her Master of Medical Science in Physician Assistant Studies at Midwestern University and is credentialed through the National Commission on Certification of Physician Assistants. She chose to work in a community health center to help increase access to quality, patient-centered care and address barriers to health. Gabby works to form meaningful relationships with her patients so that she can help them achieve their long-term health goals. Outside of work, Gabby enjoys spending time with her family and friends, long walks with her dog, and reading.  

Kelsey Carnes, P.A.

Kelsey Carnes completed her Master’s of Physician Assistant Studies at Trine University and is credentialed through the National Commission on the Certification of Physician Assistants. She is originally from Indianapolis, IN and obtained her bachelor's degree in exercise science from Purdue University. She decided to work at a community health to make health care more accessible to all individuals and increase health literacy in the community. Kelsey takes the time to get to know each of her patients to create an individualized health care plan that increases their quality of life. Outside of work, Kelsey enjoys fitness and can often be found training for her next half marathon or playing in her tennis league.

Nycole Moore, F.N.P.

Nycole Moore earned her Master's of Science in Nursing at Chamberlain University and completed her practicum at Advocate Medical Group Family Medicine. She is credentialed through the American Nurses Credentialing Center. As a nurse practitioner, Nycole values building strong, trusting patient relationships and providing patient-centered, holistic care. She is excited to work in a community health center and make an impact within medically underserved communities.  

Brea Harris, W.H.N.P.

Brea Harris received her Master of Science in Nursing from Yale University and is a certified Women’s Health Nurse Practitioner through the National Certification Corporation. Brea strives to empower her patients to make positive health choices that not only impact their overall health and well-being but create healthier communities. She provides care through a trauma informed care lens so she can provide the best care to her patients while understanding their needs. Outside of work, Brea enjoys reading, writing and running.

Carolina Ayala, P.A.-C.

Carolina Ayala, P.A.-C. received her Master of Physician Assistant Studies at Northern Arizona University and is credentialed through the National Commission on Certification of Physician Assistants. She is a member of the American Academy of Physician Assistants and the Illinois Academy of Physician Assistants. Carolina enjoys being a part of a multidisciplinary team to provide high quality patient health care in communities that need it most. She strives to deliver compassionate health care and partners with her patients to tackle health problems and goals together. Outside of work, Carolina enjoys traveling, snowboarding and finding new places to eat. She is fluent in Spanish.

Samantha Fredrickson, P.A.

Samantha Fredrickson earned her Master of Health Sciences in Physician Assistant Studies at Drexel University. She is a certified Physician Assistant through the National Commission on Certification of Physician Assistants and is a standing member of the American Academy of Physician Assistants. After completing her community health rotations, Samantha found a passion for providing fully integrated, patient-centered health care to meet her patients’ unique needs. She provides high-quality, comprehensive care that focuses on disease prevention and positive health outcomes. Outside of work, Samantha enjoys spending time outdoors, playing with her dog Daisy, and being with friends and family.

Reem I. Anz, M.D.

Reem Anz earned her medical degree at Hashemite University in Zarqa, Jordan and completed her residency at Western Michigan University School of Medicine. Dr. Anz is a member of the American Academy of Pediatrics. Dr. Anz believes that everyone deserves equal access to quality health care and is dedicated to advocating for children from all backgrounds. Outside of work, Dr. Anz can be found reading, drinking coffee, and enjoying nature. She is fluent in Arabic.

Chelsee Uhlar, W.H.N.P.

Chelsee Uhlar completed her nurse practitioner studies at Frontier Nursing University with a specialization in women’s health. She is accredited through the National Certification Corporation and is a member of Association of Women's Health, Obstetric and Neonatal Nurses, the American Academy of Nurse Practitioners, and the National Association of Nurse Practitioners in Women’s Health. Chelsee sees health care as a human right and is committed to providing high quality patient-centered care to communities with barriers to care. She takes the time to listen to all her patients' concerns and works with them to find solutions that improve their overall health. Outside of work, Chelsee enjoys traveling, hiking, and kayaking.

Jessica Rebstock, F.N.P.

Jessica Rebstock attended Resurrection University where she earned a Master in Science in Nursing Primary Care as a Family Nurse Practioner. She is credentialed through the American Academy of Nurse Practioners and is part of the Sigma Theta Tau International Honor Society of Nursing. Jessica decided to enter community health because, “family health is at the heart of community health. Healthy individuals contribute to healthy families which produce health communities.” She believes that health care is a right for all indivduals and as a nurse practioner who needs to help patients main the healthiest life possible. In her spare time she enjoys exercising, cooking and spending time with her cat and dog.

Erleine Bautista, M.D.

​Dr. Bautista received her medical degree from Georgetown University School of Medicine and completed her residency at Danbury Hospital in Danbury, CT.  She is a member of the American College of Obstetricians and Gynecologists and the American Association of Gynecologic Laparoscopists. Dr. Bautista is committed to giving back to medically underserved communities and believes that everyone deserves access to quality health care. In her spare time, Dr. Bautista enjoys cooking and traveling.

Teresa Cvengros, M.D.

Dr. Cvengros earned her medical degree at Southern Illinois University School of Medicine and completed her residency at Cook County Hospital. She is licensed by the American Board of Family Medicine and has a Certificate of Added Qualifications in Sports Medicine. Dr. Cvengros is a member of the American Academy of Family Physicians and the American College of Sports Medicine. She focuses on providing high quality care to all patients and is fluent in Spanish. Outside of work Dr. Cvengros enjoys traveling exercise and soccer.

Amanda Lee received her Master's of Science in Nursing from the Yale School of Nursing. She is licensed by the American Nurses Credentialing Center and is a member of the National Association of Pediatric Nurse Practitioners and Sigma Theta Tau International Honor Society of Nursing. Amanda works to be an advocate educator and partner for accessible quality health care for the children and families who come to the health center. She believes in empowering patients through education. When not working, Amanda enjoys traveling spending time with her friends, family, and its small herd of dairy cattle.
