Hans Lonnroth, L.C.S.W.

Hans Lonnroth received his Master of Social Work at the University of Illinois Chicago and is credentialed through the State of Illinois. Hans has experience working with patients with addiction and trauma and is committed to working through mental health challenges with patients. Hans aims to establish strong relationships with his patients built on trust and understanding. When not at the health center, Hans can be found hunting for fossils, spending time with his pets, and listening to music.  

Christy Reinhard, M.D.

Dr. Christy Reinhard is a graduate of Duke University and Rush Medical College, from which she received her medical degree. She completed her residency at Cook County Children’s Hospital, serving as chief resident of pediatrics, and spent nearly eight years doing general outpatient pediatrics for Cook County Hospital’s community health network. She is board-certified by the American Board of Pediatrics and is a member of the American Academy of Pediatrics. “Every child deserves quality health care, and every family deserves a pediatrician they feel they can trust,” she says. “I hope all my patients know that I think their concerns are extremely important. What can be more important than the health of all our children?” Dr. Reinhard and her husband have three children. In her spare time, she enjoys reading, baking, attending plays and concerts, and spending time with her family. She is fluent in English and Spanish.

Hazel Langcauon, M.D.

Hazel Langcauon earned her medical degree at the University of the Philippines and completed her residency at Mount Sinai Hospital here in Chicago. She is Board-certified in Family Medicine, Lifestyle Medicine, and Obesity Medicine and is a member of the Illinois Academy of Family Physicians, American Academy of Family Physicians, and the American College of Lifestyle Medicine. Dr. Langcauon believes that patients and physicians should work as a team and empowers her patients to engage with her in their health care and treatment plans so that they can work together to She believes that patients and physicians should work as a team to achieve patients’ health goals. Outside of work, Dr. Langcauon enjoys traveling and spending time with her family. She is fluent in Tagalog.

Toree E. Curtis, P.A.

Toree Curtis earned her Master of Medical Science in Physician Assistant Studies at the University of the Cumberlands Northern Kentucky Campus. She is Board-certified through the National Commission on Certification of Physician Assistants. Toree views herself as an advocate for her patients and works to provide affordable and accessible services in their own communities. She understands the importance of creating a safe space and building positive, trusting relationships with her patients. Toree also enjoys reading, cooking, and spending time with loved ones.

Roseline Nwokeji, A.P.R.N., F.N.P.-B.C.

Roseline Nwokeji received her Master of Science in Nursing from Olivet Nazarene University. Roseline is credentialed by the American Nurses Credentialing Center, and a member of the American Association of Nurse Practitioners and the Sigma Theta Tau honor society. Roseline is drawn to community health and preventative care after seeing too many cases of preventable illnesses during her time as an R.N. in an intensive-care unit. She believes in empowering people to better care for themselves holistically. Outside of work, Roseline enjoys traveling and she is fluent in Ibo.  

Obioma Oparanozie, M.D.

Dr. Obioma Oparanozie received her medical degree from the University of Port in Harcourt, Nigeria and completed her residency at Stroger Hospital in Chicago. Dr. Oparanozie is certified by the American Board of Obstetrics and Gynecology, and a member of the American Medical Association and the American College of Obstetricians and Gynecologists. Dr. Oparanozie believes everyone deserves the best care possible, which is what draws her to community health care and providing for the underserved communities. In her free time, Dr. Oparanozie enjoys cooking, reading, singing and spending time with her family.   

Deana Gaston, L.C.S.W.

Deana Gaston received her Master of Social Work in Science from the University of Tennessee and is credentialed through Illinois Department of Financial Professional Regulation. Deana is committed to providing culturally competent care to her patients and her philosophy of care is strength based, collaborative and nonjudgmental with a focus on treating the whole person. Outside of work, Deana enjoys reading, traveling and spending time with friends.

Shivang Shah, D.P.M.

Shivang Shah received his Doctor of Podiatric Medicine degree from the Rosalind Franklin University of Medicine and Science and is certified through the American Board of Podiatric Medicine. He completed his residency at AMITA Health Saint Joseph Hospital and is a member of the American Podiatric Medical Association, and American College of Foot and Ankle Surgeons. Dr. Shah believes in providing personalized, holistic, and compassionate care to each of his patients. He is committed to educating patients about their conditions and tries his best to provide painless, non-invasive treatments. In his spare time, Dr. Shah enjoys going on hikes, riding his bike, and photography.

Nancy Glick, M.D.

Dr. Glick received her medical degree at Rush Medical School, completed her residency in internal medicine at University of Washington in Seattle, and completed a fellowship in infectious disease at Rush University Hospital. She currently serves as medical director of HIV services for ACCESS. Dr. Glick’s passions as a physician include keeping up to date with issues concerning HIV, STDs, resistant pathogens, and hepatitis. “The best part of being a physician,” she says, “is working with and getting to learn about patient’s lives and getting to know their care and quality of life.” In her spare time, Dr. Glick can be found spending time with her family or keeping in shape through bicycling, walking, and yoga.​

Tania Nunez, L.C.P.C.

Tania Nunez earned her Master of Science in Community Counseling from National Louis University and completed her practicum at Rainbow House in Little Village working with survivors of domestic violence. She is credentialed through the Illinois Department of Financial and Professional Regulation and is a member of the Illinois Counseling Association and the Eye Movement Desensitization Reprocessing International Association. Tania chose to serve in community health after witnessing health disparities within her own community growing up. She works alongside her patients to ensure they are reaching their health goals and provides the best level of care to address their needs. Outside of work, Tania enjoys reading, listening to music, and connecting with her community. She is fluent in Spanish.  

Hemlata Meena, M.D.

Dr. Hemlata Meena received her medical degree at Maulana Azad Medical College in New Delhi and completed her residency at Mount Sinai Hospital in Chicago, Illinois. She is credentialed through the Illinois Department of Financial and Professional Regulation and is a member of the American Board of Obstetrics and Gynecology. Dr. Meena is passionate about providing women’s health care in community health care settings and helping patients to empower themselves take care of their own health. To Dr. Meena, health is our biggest asset and it must be taken care of to live happy, full lives. She believes women have a unique opportunity to be role models and significantly shape the health of their families and communities. Outside of work, she enjoys dancing, meditating, spending time with family, and interior decorating. She is fluent in Hindi.

David Sode, P.A.

David Sode graduated with his Master of Science in Physician Assistant Studies from Rosalind Franklin University. He is certified through the National Commission on Certification of Physician Assistants. David believes in meeting patients where they are at, listening with compassion and working together to empower them in their care. He takes the time to get to know each patient beyond their diagnosis, building trust and making sure they and their families feel understood and supported every step of the way. Outside of work, David enjoys spending time with family and friends, hiking, and playing the drums. He is fluent in Spanish.

Patricia J. Zoller, A.P.R.N., F.N.P.-B.C., received her Master of Science in Nursing from Saint Xavier University. She is credentialed through the American Nurses Credentialing Center and is a member of the American Association of Nurse Practitioners and Sigma Theta Tau Honors Society. Patricia strives to provide exceptional health care with an emphasis on health education and chronic disease management. She believes everyone should have access to quality health care and through education and strong relationships with patients, find the best care plan that improves health outcomes. Outside of work, Patricia enjoys spending time with her family and traveling.

Desiree Khan, F.N.P.-B.C.

Desiree Khan received her Master of Science in Nursing from Olivet Nazarene University and completed her clinical rotation at Little Company of Mary Emergency Room. She is credentialed through the American Nurses Credentialing Center (ANCC) and is a member of the American Association of Nurse Practitioners and ANCC. Desiree believes everyone deserves access to quality health care and delivers care by collaborating with patients to create care individualized care plans.

Mamata Khanal, M.D.

Dr. Khanal received her medical degree from Nepalgunj Medical College in Nepal, and completed her residency at John H. Stroger Jr. Hospital of Cook County. She is licensed through the Illinois Department of Financial and Professional Regulation (IDFPR). Dr. Khanal is passionate about working in a medically underserved community. She is committed to caring for her young patients and ensuring they receive the care they need. She is fluent in Nepali and Hindi. In her spare time, Dr. Khanal enjoys reading books and traveling.
