Dr. Delgado received his medical degree from Universidad Nacional Autonoma de Mexico (UNAM) in 1988, and completed his pediatrics residency at Columbus Hospital in 1994. As the child of first-generation, working-class, immigrant parents, Dr. Delgado says, “I feel very at home in the setting where I work. I have a complete connection with the patients I have been entrusted to serve.” He has been a fellow of the American Academy of Pediatrics since 1994. Dr. Delgado is board-certified in pediatrics and is fluent in Spanish.

Dr. Hemlata Meena received her medical degree at Maulana Azad Medical College in New Delhi and completed her residency at Mount Sinai Hospital in Chicago, Illinois. She is credentialed through the Illinois Department of Financial and Professional Regulation and is a member of the American Board of Obstetrics and Gynecology. Dr. Meena is passionate about providing women’s health care in community health care settings and helping patients to empower themselves take care of their own health. To Dr. Meena, health is our biggest asset and it must be taken care of to live happy, full lives. She believes women have a unique opportunity to be role models and significantly shape the health of their families and communities. Outside of work, she enjoys dancing, meditating, spending time with family, and interior decorating. She is fluent in Hindi.

Adriana Rivera is a Certified Nurse Midwife and completed her Masters of Nursing at the University of Illinois Chicago. She is credentialed through the American College of Nurse Midwives. Outside of work, Adriana enjoys cooking, reading, and spending time with her family. She is fluent in Spanish.

Dr. Michael enjoys being a part of and working in a community so that she can collaborate with other specialties and disciplines to ensure optimal care of her patients. As a medical provider, Dr. Gondi believes in providing high-quality, patient-centered care to all of his patients, regardless of their background. Outside of work, he enjoys exploring museums, art galleries, and travelling.

Michael Heard received his Masters in Social Work at Loyola University of Chicago and completed his practicum at Grant Hospital Adult Psychiatric Unit. He is licensed through the State of Illinois and has been a lifelong resident of Chicago. He believes patients deserve quality medical and mental health services and is grateful to have the opportunity to offer essential care services in the community. Outside of work, Michael enjoys exercising and bike riding.

His approach to care is meeting patients where they are and working together to address their personal health needs by fostering a non-judgmental and patient-centered environment. Kevin chose to work in community health care because he believes that providing high-quality accessible health care impacts communities at multiple levels. When not at work, Kevin can be found playing guitar, cooking, hiking, and making pottery. He is fluent in Spanish.

She became interested in community health after she completed a year of service as a health educator at a school-based health center in Chicago. In her practice, Dr. Hill aims to get to know her patients as people first so she can build a caring and supportive relationship. When not at work, Dr. Hill can be found playing violin or running in her neighborhood.
