Marina Ibrahim, M.M.S., P.A.-C.

Marina Ibrahim completed her Master of Medical Science in Physician Assistant Studies at Midwestern University. She is certified through the National Commission on Certification of Physician Assistants and is a member of the American Academy of Physician Assistants. In her practice, Marina provides patient-centered care and treats each patient like family. She believes in giving back to her community through preventative medicine and providing accessible, quality care to all patients. When not at work, Marina can be found exploring new places and spending time with her family and friends. She is fluent in Arabic.

Sanah Ruhullah earned her medical degree at the Shadan Institute of Medical Sciences in Hyderabad, India and completed her residency at the Puerto Rico Children’s Hospital. She decided to work in a community health care setting because she enjoys providing holistic care to diverse communities. Dr. Ruhullah believes that all children deserve access to high quality, patient-centered care. She is a self-described foodie and also enjoys gardening and traveling. Colleen Joseph, D.N.P., I.B.C.L.C.

Colleen Joseph, D.N.P., I.B.C.L.C., received her Doctorate of Nursing Practice from Loyola University. She is credentialed through the American Nurses Credentialing Center and is a member of the American Association of Nurse Practitioners, Nurse Practitioners in Women’s Health and Sigma Theta Tau Honors Society. Colleen has committed her medical career to providing care to those who need it most and believes everyone should have access to quality, affordable health care. Every patient she sees, she strives to provide them the same care and attention she would show to a member of her family. Outside of work, Colleen enjoys running marathons, CrossFit and spending time with friends and family. She is fluent in Spanish.

Peter J. Johnson, M.D.

Peter Johnson earned his medical degree from the University of Illinois-Chicago and completed his residency at the University of Hawaii-Manoa. He is a Board-certified physician in the state of Illinois and is a member of the American Board of Obstetrics and Gynecology. Dr. Johnson values serving a diverse patient population and takes a holistic, patient-centered approach to care. He wants to empower his patients to make the best decisions for their health and treats each of his patients like family. When not at work, Dr. Johnson enjoys staying active through yoga, swimming, biking, and traveling. He is fluent in Spanish.  

Ivy S. Jones, P.M.H.N.P.-B.C.

Ivy Jones earned her Masters of Science in Nursing at Walden University and completed her practicum at the University of Chicago. She is a certified Psychiatric Mental Health Nurse Practitioner and is credentialed through the American Nurses Credentialing Center. Ivy incorporates the principles of patient-centered care into her practice and treats each patient with compassion, while providing evidence-based treatments. She chooses to work in community health in order to provide high quality mental health care to medically underserved communities. When not at work, Ivy can be found fishing, reading, or spending time with her family.

Luke Agase, P.A.-C.

Luke Agase completed his Masters of Physician Assistant Studies at Lipscomb University in Nashville, Tennessee. He is credentialed through the National Commission on Certification of Physican Assistants and is a member of the American Association of Physician Assistants and the Illinois Academy of Physician Associates. In his practice, he seeks to serve people of all backgrounds with compassion and kindness. He is committed to ACCESS’ mission of providing high quality, patient centered care to all of his patients. Juanita Uribe, M.S.W., L.C.S.W

Juanita Uribe earned her Masters Degree in Social Work from Loyola University Chicago and is licensed through the Illinois Department of Financial and Professional Regulation. Juanita has spent her career serving individuals, couples, and families throughout various Chicagoland communities and specializes in helping people through difficult transitions, as well as managing anxiety, depression, and building resilience. She works collaboratively with clients to set and achieve meaningful goals and create a strong, therapeutic relationship. Outside of work, Juanita enjoys spending time with family and traveling. She is fluent in Spanish.  

Aarti Kulkarni, M.D.

Aarti Kulkarni earned her medical degree at the University of the West Indies in Kingston, Jamaica and completed her residency at Driscoll Children’s Hospital in Corpus Christi, Texas. Dr. Kulkarni also completed her Neonatal Perinatal Medicine Fellowship at the John H. Stroger Jr. Hospital of Cook County. She is Board-certified through the American Board of Pediatrics and is a member of the American Academy of Pediatrics. Dr. Kulkarni is passionate about providing high quality, patient-centered care to her patients and their families. With education, she works with her patients to ensure that they can make the best decisions for their wellness journey. Outside of work, Dr. Kulkarni enjoys Bollywood dancing and creative writing. She is fluent in Hindi, Marathi, Telugu, Kannada, and Jamaican Patois.  

Martina Krone, N.P.

Martina Krone received a Master of Science in Nursing with a specialization in Women’s Health and Gender Related Care from Vanderbilt University. Martina, also called Mia, is credentialed through the National Certification Corporation and is a member of the American Association of Nurse Practitioners, the National Association of Nurse Practitioners in Women’s Health, and Nurses in Sexual and Reproductive Health. She practices patient-centered and trauma-informed care and works to make all of her patients feel safe, understood, and empowered. She is passionate about health education and emphasizes bodily empowerment with all of her patients. When not at work, Martina can be found exploring local coffee shops, practicing yoga, and spending time by the lake.

Cynthia Lullo, P.A.

Cynthia Lullo received her degree from Midwestern University and is a credentialed Physician Assistant through the National Commission on Certification of Physician Assistants (NCCPA). She is a member of the Illinois Academy of PAs (IAPA) and the American Academy of Physician Assistants (AAPA). Cynthia believes providing education and preventive care to her patients is essential to providing the best care. Outside of work, Cynthia enjoys hiking, reading and spending time with family. She is fluent in Spanish.

Jasmine Munoz, M.D.

Dr. Jasmine Munoz received her medical degree from Rush University in Chicago and completed her residency at Advocate Lutheran General Hospital. Dr. Munoz is certified by the Illinois Department of Public Health, and a member of the American Congress of Obstetricians and Gynecologists. She believes being knowledgeable about one’s own body and health empowers people to make healthier decisions that lead to increased longevity and life fulfillment. While growing up, she witnessed how neglecting health care needs can lead to negative consequences, which is what draws her to community health and providing for underserved communities. In her free time, Dr. Munoz enjoys spending time with her family. She is fluent in Spanish.

Marie E. Schlentz, D.O.

Dr. Marie Schlentz received her medical degree from Chicago College of Osteopathic Medicine and completed her residency at Rush-Presbyterian St. Luke Medical Center, Chicago. Dr. Schlentz is a member of the American College of Physicians and is licensed by the Illinois chapter of the American Board of Internal Medicine. She is dedicated to a practice foundation of strong patient-provider relationships and preventative medicine. Dr. Schlentz encourages patient understanding, lifestyle changes, and compliance with the proper and least amount of medication for treatment. ​She is an avid reader and in her free time enjoys scuba diving, aerobic exercise with weight lifting, travel, and an overall healthy lifestyle. 

Linda Guzman-Kurani, N.P.

Linda Guzman-Kurani received her Master of Science in Nursing from the University of Illinois at Chicago. She is licensed by the Pediatric Nursing Certification Board and is a member of the National Association of Pediatric Nurse Practitioners. In her spare time, Linda enjoys traveling, dancing, swimming and testing her taste buds with new foods. Linda grew up receiving her primary care through community health, so understands first-hand the impact it can have on a patient. She is dedicated to listening to her patients, advocating for them and providing the best possible patient care. Linda is fluent in Spanish.  

Daneen Woodard, M.D.

​Dr. Woodard received her medical degree from Southern Illinois University and completed residencies in pediatrics and internal medicine at the University of Illinois at Chicago. Dr. Woodard enjoys delivering community-based health care and says she loves people and doing what she can to help those in need.​

Lisa Russell, M.D.

Dr. Lisa Russell received her medical degree from Wayne State University School of Medicine in Michigan and received her bachelor's degree from Michigan Technological University. She completed her residency at St. Joseph Mercy hospital in Ann Arbor, Michigan and fellowships at Rush-Presbyterian St. Lukes and Cook County hospitals in Chicago. Dr. Russell is licensed by the American Board of Internal Medicine and certified in Internal Medicine and Infectious Diseases. Outside of work Dr. Russell enjoys cooking reading and sports.
