Oladimeji M. Kassim, P.M.H.N.P.-B.C.

Oladimeji Kassim earned his Masters in Nursing at Northern Kentucky University. He is accredited through the American Nurses Credentialing Center and is a member of the American Association of Nurse Practitioners. Oladimeji chose to work in a community health center to become closer to his community and give back to those in need of health care. He believes in providing patient-centered, holistic care, taking the time to give each of his patients a treatment plan unique to their needs. Outside of work, Oladimeji can be found spending time with his family and staying active. He is fluent in Yoruba.

David Sode, P.A.

David Sode graduated with his Master of Science in Physician Assistant Studies from Rosalind Franklin University. He is certified through the National Commission on Certification of Physician Assistants. David believes in meeting patients where they are at, listening with compassion and working together to empower them in their care. He takes the time to get to know each patient beyond their diagnosis, building trust and making sure they and their families feel understood and supported every step of the way. Outside of work, David enjoys spending time with family and friends, hiking, and playing the drums. He is fluent in Spanish. 

Melva Navarro, P.A.

Melva Navarro completed her physician assistant studies at the Yale School of Medicine. She is credentialed through the National Commission on Certification of Physician Assistants and is a member of the American Academy of Physician Assistants. Melva is dedicated to serving the community she was raised in by providing culturally competent care that understands the factors that impact patients’ quality of care. She wants to establish authentic connections with each of her patients so that she can collaborate with them on a care plan that improves their quality of life. Outside of work, Melva enjoys spending time with her children, listening to audiobooks, and dancing. Melva was born in Veracruz, Mexico and is fluent in Spanish.

Desiree Khan, F.N.P.-B.C.

Desiree Khan received her Master of Science in Nursing from Olivet Nazarene University and completed her clinical rotation at Little Company of Mary Emergency Room. She is credentialed through the American Nurses Credentialing Center (ANCC) and is a member of the American Association of Nurse Practitioners and ANCC. Desiree believes everyone deserves access to quality health care and delivers care by collaborating with patients to create care individualized care plans.

David Vallejo, M.D.

Dr. Vallejo received his medical degree from the University of Wisconsin Madison School of Medicine and completed his residency at Mercy Hospital and Medical Center. He is fluent in Spanish and when not working, enjoys dining out cooking with family traveling and reading. Dr. Vallejo enjoys giving back to the community and he believes in a holistic health care approach.

Eunice Kim, M.D.

​Dr. Kim received her medical degree from Rush Medical College and completed the Deaconess Family Medicine Residency at Deaconess Hospital. She is board-certified by the American Board of Family Medicine, and she is a member of the American Academy of Family Physicians. Dr. Kim believes in empowering her patients to take optimum care of themselves, while seeing the patient as a person and not a medical condition. She is fluent in Korean. When not working, Dr. Kim enjoys playing the guitar, hiking, gardening, enjoying art and spending time with her family.   

Deep Patel, P.A.

Deep Patel earned his Master of Medical Science in Physician Assistant Studies from Midwestern University. He is certified by the National Commission on Certification of Physician Assistants and is an active member of the American Academy of Physician Associates and the Illinois Academy of Physician Assistants. Deep is committed to providing holistic, patient-centered care that promotes better health outcomes, with a focus on building trust and treating every patient with respect. Outside of work, Deep enjoys spending time with family and friends, watching sports, and exercising. 

Dennis Poplavskyy, P.A.

Dennis Poplavskyy earned his Master of Medical Science in Physician Assistant Studies from Midwestern University. He is certified by the National Commission on Certification of Physician Assistants and is a member of the American Academy of Physician Associates. Dennis is dedicated to providing high-quality, compassionate care to underserved populations. His philosophy of care is simple and patient-centered, focused on treating each patient as he would like to be treated, with respect, empathy, and attention to individual needs. Outside of work, Dennis enjoys reading and playing basketball. He is fluent in Russian and Ukrainian.

Sydney Yotter, P.A.

Sydney Yotter graduated from Midwestern University with a Master of Medical Science in Physician Assistant Studies. She is certified through the National Commission on Certification of Physician Assistants and is a member of the American Academy of Physician Assistants and the Illinois Academy of Physician Assistants. Sydney works to create a positive impact within the community by eliminating barriers to quality health care. She takes the time to form trusting relationships with her patients and makes their physical and mental health her priority. Outside of work, Sydney enjoys traveling, reading, and spending time with her dog.

Joy De Leoz, M.D.

Dr. De Leoz received her medical degree from the University of the East in the Philippines and completed her residency at West Virginia University. She is licensed by the American Board of Family Practice and is also a member of the American Academy of Family Physicians and the American Academy of Dermatology.

James Kim, M.D.

Dr. James Kim received his medical degree from University of Toledo, College of Medicine and completed his residency at State University of New York in Buffalo, N.Y. Dr. Kim is certified by the American Board of Obstetrics and Gynecology and a Fellow member of the American Congress of Obstetricians and Gynecologists. In his free time, Dr. Kim enjoys watching sports, playing golf and traveling. He is drawn to community health care because he believes in connecting with the community through compassionate, advanced, high-quality care.

Viviana Rivera, F.N.P.-C.

Viviana Rivera received her Master of Science in Nursing from Chamberlain College of Nursing. She is credentialed by the American Association of Nurse Practitioners. Viviana is committed to helping her patients play an active role in their health care. She believes her background has allowed her to help people overcome language and educational barriers. Her interests outside of work include spending time with her family, watching movies and playing soccer. Viviana is fluent in Spanish.

Jacqueline Greene-Roper, M.D.

Jacqueline Greene-Roper earned her medical degree at the Loyola Stritch School of Medicine and completed her residency at the West Suburban Medical Center. Dr. Greene-Roper is licensed through the Illinois Department of Financial and Professional Regulation and is a member of the American Academy of Family Physicians as well as the Illinois Academy of Family Physicians. Dr. Greene-Roper's approach to care is guided by the belief that every person deserves quality, comprehensive, and efficient health care that is built on a positive patient-physician relationship. Dr. Greene-Roper understands the difficulties many people in Chicago face in accessing care and assists her patients in providing linkages to health care and other services. Outside of work, she enjoys spending time with her family, cooking, and being in nature.

Joseph Mountain, P.A.

Joseph Mountain graduated from Rosalind Franklin University School of Medicine and Science with a Masters degree in Physician Assistant Studies. He is credentialed through the National Commission on Certification of Physician Assistants and is a member of the American Academy of Physician Assistants. Joseph is passionate about family medicine and knew that working at ACCESS would allow him to use this passion to better the lives of his patients. In his practice, Joseph uses a patient-centered approach where he looks at the patient’s physical, mental, and social health rather than only focusing on the issue presented at the visit. He believes that this approach to patient care is vital to the long term well being of his patients. When not at work, Joseph can often be found in nature, either gardening, camping, fishing, or even rock climbing.
