Elda Mullaj, M.D.

Elda Mullaj, MD received her medical degree from the Saint James School of Medicine and completed her residency at Mount Sinai Hospital through a Rosalind Franklin University of Medicine Fellowship. She is Board certified through the American Board of Internal Medicine and is a member of the American College of Cardiology. Dr. Mullaj believes that all people deserve high-quality, unbiased, and affordable care, regardless of their background. She is committed to listening deeply, honoring cultural contexts, and partnering with patients to create treatment plans that respect their values. Outside of work, Dr. Mullaj enjoys reading, cooking, and exercising. She is fluent in Albanian.  

Carla Florencio, M.D.

Carla Florencio earned her medical degree at La Universidad Iberoamericana in Dominican Republic and completed her residency at SUNY Downstate Health Sciences University in Brooklyn, New York. She is Board-certified by the American Academy of Pediatrics and is licensed through the Illinois Department of Financial and Professional Regulation. She is an active member of the American Academy of Pediatrics. Dr. Florencio is passionate about providing high quality health care in medically underserved communities. . Her goal is to guide patients to have agency over their health, promote a healthy lifestyle, while providing compassionate, high quality, patient-centered care to all of her patients and their families. Dr. Florencio enjoys cooking, traveling, and going to live music events. She is fluent in Spanish.

Lorelei Beyer, C.N.M, W.H.N.P.

Lorelei earned her nursing degree from the University of Illinois at Chicago. She is credentialed by the American Midwifery Certification Board. She is also a member of the American College of Nurse-Midwives. Lorelei recognizes the difficulty that underserved populations go through to access care, and she wants to help make a difference in that area. She believes in listening to women and tailoring their health care plans to fit their needs and lifestyles. ​When not working, Lorelei enjoys scuba diving, traveling, and sports.   

D'Andra Mixon-Walker, P.A.

D’Andra Mixon-Walker earned her Master of Science in Physician Assistant Practice at Rosalind Franklin University. She is a certified Physician Assistant through the Illinois Department of Professional and Financial Regulation and is a member of the National Commission on Certification of Physician Assistants, American Academy of Physican Assistants, and the Illinois Academy of Physician Assistants. D’Andra builds trust with her patients by being an active listener, working collaboratively and providing research-based care. She values community-based care and works to ensure her community receives holistic, high-quality, and accessible care. Outside of work, D’Andra enjoys long-distance running and live music.

Jovan Chandler, L.C.P.C.

Jovan Chandler received his master’s degree from Roosevelt College in Chicago. He is drawn to community health because he has a desire to give back and serve those around him. He feels that everyone has a voice and important perspective that deserves to be heard. Jovan’s approach to care is through consideration of the whole person and not just addressing the specific problem at hand. Outside of work, Jovan enjoys reading, traveling and sports.

Audrey Richardson, M.D.

Dr. Audrey Richardson received her medical degree from Fatima College of Medicine in the Philippines and completed her residency at St. Elizabeth’s Family Practice in Chicago. She is licensed by the American Board of Family Medicine and a member of the Illinois Academy of Family Physicians. Dr. Richardson is dedicated to serving the needs of underserved families and individuals from newborn, pediatrics, adolescents and geriatrics. She is fluent in Spanish and Tagalog. In her free time, Dr. Richardson enjoys music, movies and reading.

June Garcia-Uribe, N.P.

June Uribe received her Master of Science in Nursing from Purdue University Global and is a certified Advanced Practice Nurse through the American Academy of Nurse Practitioners. She is also a member of the American Nurses Association. In her practice, June provides patient-centered health care that is compassionate and culturally competent. She believes everyone deserves access to quality health care, regardless of their ability to pay or insurance status. Outside of work, June enjoys traveling and baking.

Daneen Woodard, M.D.

​Dr. Woodard received her medical degree from Southern Illinois University and completed residencies in pediatrics and internal medicine at the University of Illinois at Chicago. Dr. Woodard enjoys delivering community-based health care and says she loves people and doing what she can to help those in need.​

Arnulfo Delgado, M.D.

Dr. Delgado received his medical degree from Universidad Nacional Autonoma de Mexico (UNAM) in 1988, and completed his pediatrics residency at Columbus Hospital in 1994. As the child of first-generation, working-class, immigrant parents, Dr. Delgado says, “I feel very at home in the setting where I work. I have a complete connection with the patients I have been entrusted to serve.” He has been a fellow of the American Academy of Pediatrics since 1994. Dr. Delgado is board-certified in pediatrics and is fluent in Spanish.

Motaz Alshami, M.D.

Motaz Alshami graduated from the University of Sharjah in the United Arab Emirates and completed his residency at the Detroit Medical Center and Wayne State University in Detroit, Michigan. He is licensed through the Illinois Department of Financial and Professional Licensing. Dr. Alshami believes that quality behavioral health care should be available to all, regardless of their financial or social status. He understands the value of being heard and understood and meets his patients where they are at. Outside of work, Dr. Alshami enjoys reading, playing video games, and cooking. He is fluent in Arabic.

Hemlata Meena, M.D.

Dr. Hemlata Meena received her medical degree at Maulana Azad Medical College in New Delhi and completed her residency at Mount Sinai Hospital in Chicago, Illinois. She is credentialed through the Illinois Department of Financial and Professional Regulation and is a member of the American Board of Obstetrics and Gynecology. Dr. Meena is passionate about providing women’s health care in community health care settings and helping patients to empower themselves take care of their own health. To Dr. Meena, health is our biggest asset and it must be taken care of to live happy, full lives. She believes women have a unique opportunity to be role models and significantly shape the health of their families and communities. Outside of work, she enjoys dancing, meditating, spending time with family, and interior decorating. She is fluent in Hindi.

Adriana Rivera, C.N.M.

Adriana Rivera is a Certified Nurse Midwife and completed her Masters of Nursing at the University of Illinois Chicago. She is credentialed through the American College of Nurse Midwives. She believes that everyone deserves access to quality, equitable care and is dedicated to serving our Chicago communities. Outside of work, Adriana enjoys cooking, reading, and spending time with her family. She is fluent in Spanish. 

Tania Nunez, L.C.P.C.

Tania Nunez earned her Master of Science in Community Counseling from National Louis University and completed her practicum at Rainbow House in Little Village working with survivors of domestic violence. She is credentialed through the Illinois Department of Financial and Professional Regulation and is a member of the Illinois Counseling Association and the Eye Movement Desensitization Reprocessing International Association. Tania chose to serve in community health after witnessing health disparities within her own community growing up. She works alongside her patients to ensure they are reaching their health goals and provides the best level of care to address their needs. Outside of work, Tania enjoys reading, listening to music, and connecting with her community. She is fluent in Spanish.  

Sarah Michael, M.D.

Sarah Michael graduated with her medical degree from Baylor College of Medicine and completed her residency at the New York University Medical Center. She is a Board-certified psychiatrist through the American Board of Psychiatry and Neurology and specializes in child and adolescent psychiatry. She is a member of the American Psychiatric Association and the American Academy of Child and Adolescent Psychiatry. Dr. Michael enjoys being a part of and working in a community so that she can collaborate with other specialties and disciplines to ensure optimal care of her patients. In her practice, Dr. Michael takes the time to truly get to know her patients and loved ones so that she can provide compassionate, comprehensive care that addresses their goals and concerns. Outside of work, Dr. Michael enjoys baking and traveling.
