David Sode, P.A.
David Sode graduated with his Master of Science in Physician Assistant Studies from Rosalind Franklin University. He is certified through the National Commission on Certification of Physician Assistants. David believes in meeting patients where they are at, listening with compassion and working together to empower them in their care. He takes the time to get to know each patient beyond their diagnosis, building trust and making sure they and their families feel understood and supported every step of the way. Outside of work, David enjoys spending time with family and friends, hiking, and playing the drums. He is fluent in Spanish.

Dr. David Richards received his medical degree at Ohio State University and completed his residency at Rutgers University-Newark in New Jersey. He is certified by the American Board of Internal Medicine and a member of the American College of Physicians. Dr. Richards has experience working in FQHCs and believes it to be the most rewarding work a physician can do. His philosophy of care is to use evidence-based medicine to help patients achieve the highest level of health possible. Dr. Richards has a special interest in treating opioid and alcohol use disorder. Outside of work, he enjoys staying active and spending time with his family.

Patricia J. Zoller, A.P.R.N., F.N.P.-B.C., received her Master of Science in Nursing from Saint Xavier University. She is credentialed through the American Nurses Credentialing Center and is a member of the American Association of Nurse Practitioners and Sigma Theta Tau Honors Society. Patricia strives to provide exceptional health care with an emphasis on health education and chronic disease management. She believes everyone should have access to quality health care and through education and strong relationships with patients, find the best care plan that improves health outcomes. Outside of work, Patricia enjoys spending time with her family and traveling.

Desiree Khan received her Master of Science in Nursing from Olivet Nazarene University and completed her clinical rotation at Little Company of Mary Emergency Room. She is credentialed through the American Nurses Credentialing Center (ANCC) and is a member of the American Association of Nurse Practitioners and ANCC. Desiree believes everyone deserves access to quality health care and delivers care by collaborating with patients to create care individualized care plans.
Mamata Khanal, M.D.
Dr. Khanal received her medical degree from Nepalgunj Medical College in Nepal, and completed her residency at John H. Stroger Jr. Hospital of Cook County. She is licensed through the Illinois Department of Financial and Professional Regulation (IDFPR). Dr. Khanal is passionate about working in a medically underserved community. She is committed to caring for her young patients and ensuring they receive the care they need. She is fluent in Nepali and Hindi. In her spare time, Dr. Khanal enjoys reading books and traveling.

Dr. Vallejo received his medical degree from the University of Wisconsin Madison School of Medicine and completed his residency at Mercy Hospital and Medical Center. He is fluent in Spanish and when not working, enjoys dining out cooking with family traveling and reading. Dr. Vallejo enjoys giving back to the community and he believes in a holistic health care approach.
Eunice Kim, M.D.
Dr. Kim received her medical degree from Rush Medical College and completed the Deaconess Family Medicine Residency at Deaconess Hospital. She is board-certified by the American Board of Family Medicine, and she is a member of the American Academy of Family Physicians. Dr. Kim believes in empowering her patients to take optimum care of themselves, while seeing the patient as a person and not a medical condition. She is fluent in Korean. When not working, Dr. Kim enjoys playing the guitar, hiking, gardening, enjoying art and spending time with her family.
Liseth Manjarrez, M.D.
Dr. Manjarrez received her medical degree from Indiana University School of Medicine and completed her residency at Grand Rapids Medical Education Partners/Michigan State University. She is a member of the American Academy of Pediatrics and speaks fluent Spanish. In her spare time, Dr. Manjarrez enjoys playing volleyball being outdoors and going on road trips with her family. Dr. Manjarrez believes that quality health care should be available to all children as well as a medical home where all their needs are met with joy and compassion.

Eileen Baird earned her Master of Physician Assistant Studies from Chamberlain University and is certified by the National Commission on Certification of Physician Assistants (NCCPA). She also holds a Bachelor of Science in Kinesiology from the University of Wisconsin–Madison. Eileen is passionate about providing preventive health care and improving access to quality services for Chicagoland communities, especially those facing barriers to care. She believes that patient education and early intervention are essential to long-term health. Her philosophy of care focuses on building strong relationships and working collaboratively with patients to achieve their health goals. She strives to create a supportive environment where patients feel heard, respected, and empowered. Outside of work, Eileen enjoys reading, listening to music, attending live concerts, and spending time with friends.

Mini Kuncheria received her Master of Science in Nursing from Benedictine University and her Post- Master’s Family Nurse Practitioner Certificate from Olivet Nazarene University. She completed her practicum at Edward Hines Jr. Veterans Administration Hospital. Mini is board-certified as a Family Nurse Practitioner by the American Nurses Credentialing Center (ANCC) and the American Academy of Nurse Practitioners (AANP). Mini is a member of the American Association of Nurse Practitioners, and the Academy of Medical-Surgical Nurses. In addition, she is a visiting professor at Chamberlain College of Nursing. Mini is committed to being a compassionate provider and she believes in focusing on preventative care services for the community. She is a strong believer in healthy living and is passionate about promoting healthy lifestyle choices to others. In her free time, Mini enjoys reading, cooking, and spending time with family.

Toree Curtis earned her Master of Medical Science in Physician Assistant Studies at the University of the Cumberlands Northern Kentucky Campus. She is Board-certified through the National Commission on Certification of Physician Assistants. Toree views herself as an advocate for her patients and works to provide affordable and accessible services in their own communities. She understands the importance of creating a safe space and building positive, trusting relationships with her patients. Toree also enjoys reading, cooking, and spending time with loved ones.

Roseline Nwokeji received her Master of Science in Nursing from Olivet Nazarene University. Roseline is drawn to community health and preventative care after seeing too many cases of preventable illnesses during her time as an R.N. in an intensive-care unit.
