We have an integrated approach to care and our specialists work directly with our primary care teams to ensure that all aspects of a patient’s health are reviewed and monitored on a regular basis.

Aaima Sayed, M.D.
Aaima Sayed received her medical degree from Stritch School of Medicine at Loyola University and completed her residency at Loyola University Medical Center. She is licensed through the Illinois Department of Financial and Professional Regulation. Dr. Sayed decided to work in a community health center to reduce the barriers for patients seeking behavioral health care services. She is a compassionate psychiatrist and always keeps her patients' best interests in mind. When not at work, Dr. Sayed enjoys spending time with her friends and family, trying new restaurants, and binging her favorite shows on Netflix. She is fluent in Urdu.

June Garcia-Uribe, N.P.
June Uribe received her Master of Science in Nursing from Purdue University Global and is a certified Advanced Practice Nurse through the American Academy of Nurse Practitioners. She is also a member of the American Nurses Association. In her practice, June provides patient-centered health care that is compassionate and culturally competent. She believes everyone deserves access to quality health care, regardless of their ability to pay or insurance status. Outside of work, June enjoys traveling and baking.
Arunima Saxena, M.D.
Arunima Saxena earned her medical degree at the Medical School of Jipmer in Puducherry, India and completed her residency at Mount Sinai Hospital in Chicago. She is Board-certified through the American Board of Internal Medicine. Dr. Saxena was drawn to community health because she wanted to provide high quality care to underserved communities in Chicago. She uses a patient-centered approach to care that prioritizes long-term health as well as preventative care. Outside of work, Dr. Saxena enjoys spending time with her family. She is fluent in Hindi and Tamil.
Rachel Unger, P.A.
Rachel Unger completed her physician assistant studies at Yale University and is credentialed through the National Commission on Certification of Physician Assistants. Rachel takes a patient-centered, holistic approach to her practice and considers all aspects of a patient’s life and health, including physical, emotional, and social factors, while developing a care plan. She builds active relationships with her patients and collaborates with them to develop approachable health plans that fit into their lifestyle and benefit their overall wellbeing. Outside of work, Rachel enjoys spending time with her children and is curently learning how to play tennis.
Madison Doyle, D.N.P.
Madison Doyle earned her Doctorate in Nursing Practice, Pediatric Primary Care Program at the University of Illinois at Chicago. She is Board-certified through the Pediatric Nursing Certification Board and licensed through the Illinois Department of Financial and Professional Regulation. Madison wanted to work in community health care because she is passionate about reducing barriers to health while providing quality, patient-centered health care within her community. As a nurse practitioner, she values shared decision making with her patients and works to build strong relationships built on trust. Outside of work, Madison enjoys spending time with her husband and two young children, as well as traveling.

Louise Judge graduated with her Masters in Physician Assistant Studies from Touro University Nevada. She is credentialed through the National Commission on Certification of Physician Assistants and is a member of the American Academy of Physician Assistants. She enjoys working with a diverse group of people and giving back to her community. In her practice, she works to meet her patients where they are, to provide high quality, holistic care. When not at work, Louise can be found hiking, traveling, and trying new recipes.
Andres Mafla, M.D.
Dr. Andres Mafla earned his medical degree from Universidad del Valle in Cali, Columbia and completed his residency at Saint Francis Hospital in Evanston, Illinois. Dr. Mafla is a member of the American College of Physicians and has been an internal medicine specialist at Access Community Health Network for twelve years. Among his leadership roles at ACCESS, Dr. Mafla serves as a Regional Medical Director and is Chair of the Chronic Disease Committee. For the past three years, Dr. Mafla worked with the committee to improve ACCESS’s colorectal cancer screening rate from 13% to 45%. Through his leadership and diligent work on improving company policies, clinical workflows and changes to our electronic medical record (Epic), ACCESS has been able to significantly impact patient outcomes and develop best practices.
Lisa A. Neigebauer, C.N.M.
Lisa Neigebauer earned her master’s degree at Resurrection University and completed her post-master's graduate certificate at Frontier Nursing University. She is certified through the American Midwifery Certification Board and is a member of the American College of Nurse Midwives, the Illinois Society for Advanced Practice Nursing, the National Association of Nurse Practitioners in Women’s Health, and the Association of Women’s Health, Obstetric and Neonatal Nurses. After serving as a labor and delivery nurse for 20 years, Lisa decided to pursue a career in nurse midwifery. She and her patients work together as a team to create an individualized, holistic care plan throughout their pregnancy. Lisa is dedicated to eliminating barriers to health and helping grow healthier families and stronger communities. Outside of work, Lisa enjoys gardening, roller skating, and playing baseball with her children.

Gabby Alessia earned her Master of Medical Science in Physician Assistant Studies at Midwestern University and is credentialed through the National Commission on Certification of Physician Assistants. She chose to work in a community health center to help increase access to quality, patient-centered care and address barriers to health. Gabby works to form meaningful relationships with her patients so that she can help them achieve their long-term health goals. Outside of work, Gabby enjoys spending time with her family and friends, long walks with her dog, and reading.

Mario Oritz completed his Master of Science in Physician Assistant Practice at Rosalind Franklin University and is credentialed through the National Commission on the Certification of Physician Assistants. He chose to work in a community health center because he believes health care should be accessible to everyone, regardless of patients’ insurance status or socioeconomic background. He treats his patients the way he would want his family to be treated. As a husband and father of three, he understands how important it is to build strong, quality relationships with his patients. He treats each patient with kindness, respect, and dedication so that they are empowered to make the best decisions for their health and well-being. Outside of work, Mario enjoys spending time with his family, staying active outdoors, and playing board games. He is fluent in Spanish.
