Rachel Unger, P.A.
Rachel Unger completed her physician assistant studies at Yale University and is credentialed through the National Commission on Certification of Physician Assistants. Rachel takes a patient-centered, holistic approach to her practice and considers all aspects of a patient’s life and health, including physical, emotional, and social factors, while developing a care plan. She builds active relationships with her patients and collaborates with them to develop approachable health plans that fit into their lifestyle and benefit their overall wellbeing. Outside of work, Rachel enjoys spending time with her children and is curently learning how to play tennis.
Louise Judge, P.A.
Louise Judge graduated with her Masters in Physician Assistant Studies from Touro University Nevada. She is credentialed through the National Commission on Certification of Physician Assistants and is a member of the American Academy of Physician Assistants. She enjoys working with a diverse group of people and giving back to her community. In her practice, she works to meet her patients where they are, to provide high quality, holistic care. When not at work, Louise can be found hiking, traveling, and trying new recipes.
Andres Mafla, M.D.
Dr. Andres Mafla earned his medical degree from Universidad del Valle in Cali, Columbia and completed his residency at Saint Francis Hospital in Evanston, Illinois. Dr. Mafla is a member of the American College of Physicians and has been an internal medicine specialist at Access Community Health Network for twelve years. Among his leadership roles at ACCESS, Dr. Mafla serves as a Regional Medical Director and is Chair of the Chronic Disease Committee. For the past three years, Dr. Mafla worked with the committee to improve ACCESS’s colorectal cancer screening rate from 13% to 45%. Through his leadership and diligent work on improving company policies, clinical workflows and changes to our electronic medical record (Epic), ACCESS has been able to significantly impact patient outcomes and develop best practices.
Lisa A. Neigebauer, C.N.M.
Lisa Neigebauer earned her master’s degree at Resurrection University and completed her post-master's graduate certificate at Frontier Nursing University. She is certified through the American Midwifery Certification Board and is a member of the American College of Nurse Midwives, the Illinois Society for Advanced Practice Nursing, the National Association of Nurse Practitioners in Women’s Health, and the Association of Women’s Health, Obstetric and Neonatal Nurses. After serving as a labor and delivery nurse for 20 years, Lisa decided to pursue a career in nurse midwifery. She and her patients work together as a team to create an individualized, holistic care plan throughout their pregnancy. Lisa is dedicated to eliminating barriers to health and helping grow healthier families and stronger communities. Outside of work, Lisa enjoys gardening, roller skating, and playing baseball with her children.
Gabrielle Alessia, P.A.
Gabby Alessia earned her Master of Medical Science in Physician Assistant Studies at Midwestern University and is credentialed through the National Commission on Certification of Physician Assistants. She chose to work in a community health center to help increase access to quality, patient-centered care and address barriers to health. Gabby works to form meaningful relationships with her patients so that she can help them achieve their long-term health goals. Outside of work, Gabby enjoys spending time with her family and friends, long walks with her dog, and reading.
Reem I. Anz, M.D.
Reem Anz earned her medical degree at Hashemite University in Zarqa, Jordan and completed her residency at Western Michigan University School of Medicine. Dr. Anz is a member of the American Academy of Pediatrics. Dr. Anz believes that everyone deserves equal access to quality health care and is dedicated to advocating for children from all backgrounds. Outside of work, Dr. Anz can be found reading, drinking coffee, and enjoying nature. She is fluent in Arabic.
Liaht Bauer, P.A.
Liaht Bauer graduated from Southern Illinois University with a Master of Science in Physician Assistant Studies. She is credentialed through the National Commission on Certification of Physician Assistants and is a member of the American Academy of Physician Assistants. Through high-quality, patient-centered care, Liaht is able to positively impact the lives of her patients. She believes in creating strong relationships with her patients by always listening to their concerns and treating them with dignity and respect. Outside of work, Liaht is a classically trained violinist and enjoys listening to music, spending time with friends, and watching movies. Liaht is fluent in French.
Urvi Gandhi, F.N.P.
Urvi has a Masters in Science in Nursing from Saint Xavier University with a specialty in Family Nursing. She is credentialed through the American Nurses Credentialing Center and belongs to the American Association of Nurse Practitioners. She looks forward to treating and serving a diverse population with multiple conditions, making her overall become a better provider. Urvi believes a patient should be treated holistically based on their physical, emotional, psychological and spirtual needs. By working for Access she believes she will be able to give back to her community. In addition to English, Urvi is fluent in Gujurati and Hindi. Outside of work, her hobbies include watching romantic movies, being adventurous with her children and traveling.
Nadia Ilhardt, M.D.
Nadia Ilhardt earned her medical degree from Rosalind Franklin University of Medicine and Science and completed her residency at the University of Michigan. She is Board-Certified through the American Board of Family Medicine and is a member of the American Academy of Family Physicians. Dr. Ilhardt recognizes how confusing the American healthcare system can be and works with patients to navigate the system and provide high-quality care. She believes that everyone deserves patient-centered, holistic care in order to maximize their overall health. Outside of medicine, Dr. Ilhardt enjoys time with her family and spending as much time as possible outdoors.
Brea Harris, W.H.N.P.
Brea Harris received her Master of Science in Nursing from Yale University and is a certified Women’s Health Nurse Practitioner through the National Certification Corporation. Brea strives to empower her patients to make positive health choices that not only impact their overall health and well-being but create healthier communities. She provides care through a trauma informed care lens so she can provide the best care to her patients while understanding their needs. Outside of work, Brea enjoys reading, writing and running.
Jonathon Creary, M.D.
Jonathon Creary received his medical degree from the University of Medicine and Health Sciences and completed his family medicine residency at Michigan State University. He is credentialed through and a member of the American Academy of Family Physicians. Dr. Creary is committed to delivering personalized care to each patient while creating an environment that is safe, respectful and welcoming for all, particularly in regards to discussing mental health and sexual health. He is originally from Canada, but has considered Chicago home for many years now and loves to explore the city.
Colleen Tapia, D.N.P., C.P.N.P.-P.C., R.N., B.S.N., C.P.N.
Colleen earned her degree from the Rush University Doctor of Nursing Practice program. She completed her training at Advanced Pediatrics and belongs to the American Association of Nurse Practitioners. Colleen decided to work in community health because she wanted to belong to a team that works together to reduce health disparities while increasing access to care in the community. In her spare time, she enjoys spending time with her family and friends.
Erleine Bautista, M.D.
Dr. Bautista received her medical degree from Georgetown University School of Medicine and completed her residency at Danbury Hospital in Danbury, CT. She is a member of the American College of Obstetricians and Gynecologists and the American Association of Gynecologic Laparoscopists. Dr. Bautista is committed to giving back to medically underserved communities and believes that everyone deserves access to quality health care. In her spare time, Dr. Bautista enjoys cooking and traveling.
Leo Boler, M.D.
Born in Arkansas, Dr. Boler earned his medical degree from Rush Medical College in Chicago. A board-certified obstetrician and gynecologist, Dr. Boler is an experienced community-based health care provider. A member of several medical organizations, Dr. Boler has also published several articles on obstetrics and gynecology as well as contributed to academic texts on maternal medicine. Dr. Boler enjoys working with ACCESS' Healthy Start Program to help reduce infant mortality and low birth weight babies on Chicago's West Side. He lives in Chicago with his wife.
Patricia Benitez, M.D.
Dr. Benitez is a native of Cebu City Philippines and obtained her medical education at the Southwestern University School of Medicine in the Philippines. Beginning in 1977, Dr. Benitez has practiced community health care in Chicago at Palos Community Hospital, Mount Sinai Hospital and now at ACCESS South State Family Health Center.
