LaShawnda High, F.N.P.
LaShawnda High earned her Master of Science in Nursing at Olivet University. She is a member of Sigma Theta Tau and licensed through the American Nurses Credentialing Center. LaShawnda was drawn to community health care because she wanted to provide high quality, patient-centered care to communities that have historically been medically underserved. She prioritizes building strong relationships with her patients and treats each once like family. Outside of work, LaShawnda enjoys reading and spending time with friends and family.

Mary Winokur received her Masters in Physician Assistant Studies at Carroll University and is a certified Physician Assistant by the National Commission On Certification of Physician Assistants (NCCPA). She is a member of the American Academy of Physician Assistants and the Illinois Academy of Physician Assistants . Mary believes everyone deserves access to quality health care. She delivers care to her patients as if they are family with open communication so that she can build trust and work with them to create better health outcomes. Outside of work, Mary enjoys hiking, baking and spending time with family and friends.

Bridget Adams, PA-C, received her Master’s of Science in Physician Assistant studies at New York Institute of Technology and completed her residency in New York. Bridget is credentialed by the National Commission on Certification of Physician Assistants (NCCPA) and is a licensed physician assistant in Illinois. She is also a member of the American Academy of Physician Assistants. Bridget believes in providing exceptional, individualized care to her patients and chose community health care to positively impact the lives of patients and communities. Bridget played basketball through college and enjoys spending time with family and exercising.

Angeli Patel received her advanced practice registered nursing degree and completed her residency at Loyola University Chicago. Angeli is credentialed by the American Association of Nurse Practitioners. Angeli is drawn to community health because she believes in helping people by promoting health and wellness to prevent disease. In her free time, Angeli enjoys working out and spending time with family and she is fluent with Gujarati.
Susana Alvarez, M.D.
Dr. Alvarez earned her medical degree from the Universidad Francisco Marroquin in Guatemala and completed her residency at Howard University Hospital in Washington D.C. She is a member of the American Academy of Pediatrics and is fluent in both Spanish and French. Dr. Alvarez focuses on community involvement in her practice and outside of work enjoys learning other languages and sports.

Sarah Verbeek completed the Physician Assistant program at Rosalind Franklin University/Chicago Medical School. Sarah enjoys working in a community setting because she can offer holistic as well as preventive care to her patients. In her spare time, Sarah enjoys photography, travel, and tennis. Dr. Trainor received her medical degree from Southern Illinois University School of Medicine and completed a residency at Rush University Medical Center. She is a member of the American Academy of Pediatrics. In her spare time, Dr. Trainor enjoys travel, reading, listening to music, and spending time with family and friends. Dr. Trainor is fluent in Spanish and is passionate about working in a medically underserved community. She is committed to watching her young patients grow up to be happy healthy young adults.
Juanita Uribe, M.S.W., L.C.S.W
Juanita Uribe earned her Masters Degree in Social Work from Loyola University Chicago and is licensed through the Illinois Department of Financial and Professional Regulation. Juanita has spent her career serving individuals, couples, and families throughout various Chicagoland communities and specializes in helping people through difficult transitions, as well as managing anxiety, depression, and building resilience. She works collaboratively with clients to set and achieve meaningful goals and create a strong, therapeutic relationship. Outside of work, Juanita enjoys spending time with family and traveling. She is fluent in Spanish.
Aarti Kulkarni, M.D.
Aarti Kulkarni earned her medical degree at the University of the West Indies in Kingston, Jamaica and completed her residency at Driscoll Children’s Hospital in Corpus Christi, Texas. Dr. Kulkarni also completed her Neonatal Perinatal Medicine Fellowship at the John H. Stroger Jr. Hospital of Cook County. She is Board-certified through the American Board of Pediatrics and is a member of the American Academy of Pediatrics. Dr. Kulkarni is passionate about providing high quality, patient-centered care to her patients and their families. With education, she works with her patients to ensure that they can make the best decisions for their wellness journey. Outside of work, Dr. Kulkarni enjoys Bollywood dancing and creative writing. She is fluent in Hindi, Marathi, Telugu, Kannada, and Jamaican Patois.
Martina Krone, N.P.
Martina Krone received a Master of Science in Nursing with a specialization in Women’s Health and Gender Related Care from Vanderbilt University. Martina, also called Mia, is credentialed through the National Certification Corporation and is a member of the American Association of Nurse Practitioners, the National Association of Nurse Practitioners in Women’s Health, and Nurses in Sexual and Reproductive Health. She practices patient-centered and trauma-informed care and works to make all of her patients feel safe, understood, and empowered. She is passionate about health education and emphasizes bodily empowerment with all of her patients. When not at work, Martina can be found exploring local coffee shops, practicing yoga, and spending time by the lake.
Daneen Woodard, M.D.
Dr. Woodard received her medical degree from Southern Illinois University and completed residencies in pediatrics and internal medicine at the University of Illinois at Chicago. Dr. Woodard enjoys delivering community-based health care and says she loves people and doing what she can to help those in need.

She is licensed by the Pediatric Nursing Certification Board and is a member of the National Association of Pediatric Nurse Practitioners. In her spare time, Linda enjoys traveling, dancing, swimming and testing her taste buds with new foods. Linda grew up receiving her primary care through community health, so understands first-hand the impact it can have on a patient. Dr. Marie Schlentz received her medical degree from Chicago College of Osteopathic Medicine and completed her residency at Rush-Presbyterian St. Luke Medical Center, Chicago. Dr. Schlentz is a member of the American College of Physicians and is licensed by the Illinois chapter of the American Board of Internal Medicine. She is dedicated to a practice foundation of strong patient-provider relationships and preventative medicine. Dr. Schlentz encourages patient understanding, lifestyle changes, and compliance with the proper and least amount of medication for treatment. She is an avid reader and in her free time enjoys scuba diving, aerobic exercise with weight lifting, travel, and an overall healthy lifestyle.

Shahzeb Najam graduated from Ziauddin University in Pakistan and completed his internal medicine residency at Weiss Memorial Hospital in Chicago. Dr. Najam is board-certified by the American Board of Internal Medicine, fully licensed by the Illinois Department of Financial and Professional Regulation, and is a member of the American College of Physicians. He is an evidence-based clinician who continually educates himself on best practices and new technology within the medical field. Dr. Najam treats his patients like family and makes sure that they feel heard and that all of their needs are met. He works with patients to create individualized plans that meet their health goals. Outside of work, Dr. Najam is an avid reader and guitar player. He is fluent in Urdu.
