Wesley Harmston, D.O.
Dr. Wesley Harmston received his medical degree from Midwestern University and completed his residency at Resurrection Medical Center. Dr. Harmston is board-certified through the American Osteopathic Board of Family Physicians. He is a member of the Illinois Osteopathic Association, the American Osteopathic Association and the American College of Osteopathic Family Physicians. Dr. Harmston believes in providing considerate care and thoughtful interactions with all his patients. Mary Winokur received her Masters in Physician Assistant Studies at Carroll University and is a certified Physician Assistant by the National Commission On Certification of Physician Assistants (NCCPA). She is a member of the American Academy of Physician Assistants and the Illinois Academy of Physician Assistants . Mary believes everyone deserves access to quality health care. She delivers care to her patients as if they are family with open communication so that she can build trust and work with them to create better health outcomes. Outside of work, Mary enjoys hiking, baking and spending time with family and friends.
Briana Trainor, M.D.
Dr. Trainor received her medical degree from Southern Illinois University School of Medicine and completed a residency at Rush University Medical Center. She is a member of the American Academy of Pediatrics. In her spare time, Dr. Trainor enjoys travel, reading, listening to music, and spending time with family and friends. Dr. Trainor is fluent in Spanish and is passionate about working in a medically underserved community. She is committed to watching her young patients grow up to be happy healthy young adults.
Holly Perez, M.D.
Holly Perez earned her medical degree from the University of Florida and completed her residency at Northwestern University. She is a licensed physician through the State of Illinois and is a member of the American Academy of Family Physicians. Dr. Perez was drawn to community health care because she wanted to be part of a team that supports patients’ total body health. She wants her patients to feel safe to be themselves and empowered to reach their health goals with the help of a patient-centered relationship. Outside of work, Dr. Perez enjoys spending time with family and friends and traveling to Spanish speaking countries with her husband. She is fluent in Spanish.
Chirag Patel, N.P.
Chirag Patel received his Master of Science in Nursing from DePaul University in Chicago where he also earned a post-master's certificate in advanced practice nursing. He is licensed by the American Nurses Credentialing Center and a member of the Illinois Society for Advanced Practice Nursing, the Illinois Nurses Association, and the American Nurses Association. He is dedicated to serving the needs of the community and providing high-quality care in order to improve individual’s wellbeing. As a provider, it’s important to Chirag that he develops a respectful and trusting partnership with patients in order to help them make informed decisions toward healthier lifestyles.
Chirag is fluent in both Gujurati and Spanish.
Jacob Meyer, M.D.
Dr. Jacob Meyer is a general Obstetrician and Gynecologist that focuses on minimally invasive gynecologic surgery and obstetrics. Dr. Meyer received his medical degree from Oregon Health and Science University and completed a Family Medicine residency at Contra Costa Regional Medical Center in California and, later, a residency in Obstetrics and Gynecology at Oregon Health and Science University.
Dr. Meyer has a focused practice designation from the American Board of Obstetrics and Gynecology in minimally invasive surgery. His surgical focus is on laparoscopic hysterectomy, vaginal hysterectomy, laparoscopic myomectomy, advanced hysteroscopy, pelvic organ prolapse surgery, stress incontinence surgery as well as recently branching out into V-NOTES and robotic surgery. He also has experience in high-risk obstetrics, taking care of patients with type 2 diabetes, hypertension, advanced maternal age, and multiple C-sections.
Dr. Meyer is enthusiastic about providing high quality medical care to all, regardless of their insurance status, and is dedicated to engaging patients in their birth plans so they have the birth experience they prefer. He is also passionate about empowering women to take control of their reproductive health and will collaborate with them to find the birth control method that is right for them.
In his spare time, Dr. Meyer enjoys spending time with his wife and two kids, coaching t-ball, running, going to White Sox and Bulls games, fishing, camping, and traveling. Dr. Meyer is fluent in Spanish.

Chris Demers received their Master of Science in Nurse Midwifery at Georgetown University and completed their rotations at Northshore Health Systems in Lake Station, Indiana. Chris is certified through the American Midwifery Certification Board and the National Certification Corporation. They are a member of the American College of Nurse Midwives and the National Association of Nurse Practitioners in Women's Health. Chris chose to work at a community health center to provide patient-centered and holistic care within their community. Their philosophy of care centers on the belief that providers give patients the tools they need to make their own decisions. Chris enjoys cooking, going to the beach, and spending time with their cats.
Tania Nunez, L.C.P.C.
Tania Nunez earned her Master of Science in Community Counseling from National Louis University and completed her practicum at Rainbow House in Little Village working with survivors of domestic violence. She is credentialed through the Illinois Department of Financial and Professional Regulation and is a member of the Illinois Counseling Association and the Eye Movement Desensitization Reprocessing International Association. Tania chose to serve in community health after witnessing health disparities within her own community growing up. She works alongside her patients to ensure they are reaching their health goals and provides the best level of care to address their needs. Outside of work, Tania enjoys reading, listening to music, and connecting with her community. She is fluent in Spanish.
