Additionally, she is deeply interested in global health and completed her doctoral project within HIV positive communities in the Dominican Republic. When not at work, Chelsea enjoys spending time with friends and family, reading, and traveling.

Zehra Kanji-Ghaziani received her Doctorate in Nursing Practice in Family Medicine at DePaul University and also holds a Masters of Science in Nursing. She is credentialed through the American Nurses Credentialing Center. Prior to receiving her doctorate, Zehra was a labor and delivery nurse. She is committed to providing patient-centered, holistic, and equitable care to all her patients. Zehra is drawn to community health care because she wants to ensure that all members of her community have access to care. When not at work, Zehra enjoys spending time with her husband, Esmael, and their dog, Railey. She is fluent in English, Hindi, and Urdu.

Sydney Yotter, P.A.

Sydney Yotter graduated from Midwestern University with a Master of Medical Science in Physician Assistant Studies. She is certified through the National Commission on Certification of Physician Assistants and is a member of the American Academy of Physician Assistants and the Illinois Academy of Physician Assistants. Sydney works to create a positive impact within the community by eliminating barriers to quality health care. She takes the time to form trusting relationships with her patients and makes their physical and mental health her priority. Outside of work, Sydney enjoys traveling, reading, and spending time with her dog.

Dr. De Leoz received her medical degree from the University of the East in the Philippines and completed her residency at West Virginia University. She is licensed by the American Board of Family Practice and is also a member of the American Academy of Family Physicians and the American Academy of Dermatology.

James Kim, M.D.

Dr. James Kim received his medical degree from University of Toledo, College of Medicine and completed his residency at State University of New York in Buffalo, N.Y. Dr. Kim is certified by the American Board of Obstetrics and Gynecology and a Fellow member of the American Congress of Obstetricians and Gynecologists. In his free time, Dr. Kim enjoys watching sports, playing golf and traveling. He is drawn to community health care because he believes in connecting with the community through compassionate, advanced, high-quality care.

She is credentialed by the American Association of Nurse Practitioners. Viviana is committed to helping her patients play an active role in their health care. She believes her background has allowed her to help people overcome language and educational barriers. Her interests outside of work include spending time with her family, watching movies and playing soccer. Viviana is fluent in Spanish.

Amela Dedic earned her Master in Physician Assistant Studies at Northern Arizona University and is credentialed by the National Commission on Certification of Physician Assistants. She is also a member of the American Academy of Physician Assistants. Amela is passionate about working with patients and their families to address health concerns and prevent disease. She believes that all patients should have access to high quality health care and a have a team of health care professionals behind them to work together in their best interests. She looks forward to working with patients and their families to create individualized health plans and build meaningful relationships. Outside of work, she enjoys fitness, reading, and cooking. She is fluent in Bosnian.

She is licensed by the state of Illinois and a member of the American Association of Nurse Practitioners. The American Academy of HIV Medicine and the Association of Nurses in AIDS Care. Ellen believes everyone should have access to quality health care that is individualized, non-judgmental and empathetic.
