Michelle Toledo completed her Master of Physician Assistant Studies at San Juan Bautista School of Medicine. She is certified through the National Commission on the Certification of Physician Assistants and is a member of the Physician Associates for Latino Health. Having grown up on the South Side of Chicago, she has firsthand experience with the health care challenges these communities face. Her practice is centered on providing compassionate, patient-centered, and culturally sensitive care that goes beyond treating physical symptoms to include patients’ mental health and wellness. Her goal is to build trust and empower her patients to actively participate in their health journey. When not at work, Michelle can be found cooking, reading, or spending time with family and friends. She is fluent in Spanish.

Kara Egan completed her Masters in Physician Assistant Studies at Dominican University in River Forest, Illinois and is credentialed through the National Commission on Certification of Physician Assistants. Kara believes in the importance of shared decision making and prioritizes education and health literacy with her patients. Her goal is to create strong relationships with her patients that establish trust and provide them the best quality care. Tara Kristiansen received her Master of Science in Nursing at Spalding University in Louisville, Kentucky. She is credentialed through the American Academy of Nurse Practitioners and is a current member of the Illinois Society for Advance Practice Nursing. Tara is eager to work with the West Chicago community to provide high quality, patient-centered care. She believes that everyone deserves access to care, regardless of their socioeconomic background. Outside of work, Tara enjoys coaching others on budgeting and saving, as well as staying active and travelling.
Sanah Ruhullah, M.D.
Sanah Ruhullah earned her medical degree at the Shadan Institute of Medical Sciences in Hyderabad, India and completed her residency at the Puerto Rico Children’s Hospital. Dr. Ruhullah is licensed through the Illinois Department of Financial & Professional Regulation and is a member of the American Academy of Pediatrics. She decided to work in a community health care setting because she enjoys providing holistic care to diverse communities. Dr. Ruhullah believes that all children deserve access to high quality, patient-centered care. She is a self-described foodie and also enjoys gardening and traveling. She is fluent in Spanish and Urdu.
