Oral health is very important to your overall health. While ACCESS does not offer dental care directly, we believe that your oral health is a critical part of your overall health. Get Your Annual Flu Shot

The flu can strike suddenly and spreads through coughing, sneezing, and close contact. Symptoms vary, but can include fever, chills, sore throat, muscle aches, fatigue, cough, headaches and a runny or stuffy nose.

A flu vaccine is needed every flu season. Each year, a new flu vaccine is made to protect against viruses that are likely to cause disease in the upcoming flu season. Even when the year’s vaccine doesn’t exactly match the viruses, it can still provide protection.  

It takes about two weeks for protection to develop after vaccination, and protection lasts through the flu season. Visit one of our 35 ACCESS health centers located throughout the greater Chicagoland area for your flu vaccine.

 

Dedicated Care Coordination

ACCESS’ care coordination team helps those patients most at risk to stay on track with their care plan and goals of care. Care coordination is a free service offered to ACCESS patients who qualify to support their health goals. Our team members work both within ACCESS health centers and at select hospital partner sites to ensure our patients get the right care and resources when they need it.

Whether it’s diabetes, high blood pressure management, smoking cessation, weight loss or other health issues that need regular support, our care coordinators are here to provide support and guidance to eligible patients between visits.
